The physical activity levels and preferences of South African breast cancer survivors : a pilot study

- Description: Introduction: Breast cancer is currently the most commonly diagnosed cancer in South African women. Physical activity has proven to have preventative, treatment and management benefits for breast cancer and other cancers and exercise has been found as both viable and safe during cancer treatment and recovery. However, there is limited research on breast cancer and the levels and preferences of physical activity and sedentary behaviour in a South African context. Therefore, the purpose of this study was to identify levels of physical activity and sedentary behaviour of South African breast cancer survivors and to investigate the physical activity advice and participation preferences of these participants. Methods: A cross-sectional research design was implemented to identify the physical activity, sedentary behaviour levels and exercise preferences of 48 South African breast cancer survivors (age range 45 years). An online survey comprising demographic and anthropometric questions, the Godin leisure-time activity questionnaire (GLTPAQ), the International Physical Activity Questionnaire (IPAQ) and an exercise preference questionnaire was presented to participating breast cancer survivors in order to i) identify the levels of physical activity and sedentary behaviour engaged in, ii) obtain demographic and anthropometric information and iii) identify exercise preferences. A linear mixed model regression was used to examine potential associations between demographic and anthropometric variables and physical activity levels. Chi-squared and Pearson’s Product-Moment correlation tests were used to identify relationships between categorical and numerical variables. A correlation matrix was generated to further explore any correlations. Statistical significance for all measures was set at p<0.05. Results: The mean age of the group was 49 ± 9.87 years. The most common time since diagnosis was <5 years ago and the most common stage of breast cancer was stage I. The mean BMI was 27.87 ± 5.53kg/m2. The most common treatment combination was surgery with either chemotherapy or radiation. According to the leisure score index (LSI) the majority of the group (56%) was active and according to IPAQ data 60% were meeting physical activity guidelines. The highest physical activity levels were seen in the average weekly minutes of moderate-intensity activity, and there was a strong, non-significant positive correlation (p>0.05, R2 = 0.95) between moderate-intensity physical activity and total physical activity levels. High levels of weekly sedentary behaviour and sitting time (302.60 ± 169.96 minutes) were reported. A weak, non-significant, positive correlation was found between total sedentary time and BMI (p>0.05, R2 = 0.1). A weak, non-significant, negative correlation was found between age and sedentary time (p>0.05, R2 = 0.002). More participants below 50 years were insufficiently active compared to above the age of 50 years. 1.7 to 2.6 years since diagnosis saw the greatest number of insufficiently active survivors and the category over 2.6 years since diagnosis saw the most active survivors. Most breast cancer survivors (71.10% & 82.05%) indicated being interested in and feeling capable of participating in an exercise programme (p>0.05, R2 = 0.72). The favoured preference for receiving physical activity advice was face-to-face with an exercise specialist at a cancer centre before treatment. Participation preferences included starting a programme immediately after treatment, in a home-based setting with one or two other people, where walking and a moderate exercise intensity were the preferred exercise type and level of intensity. Data collection occurred both immediately prior to (42% of participants) and during (56% of participants) the South African Covid-19 lockdown, so the results should be seen in light of this context. Conclusion: The current study is one of the first to explore physical activity rates and preferences of South African breast cancer survivors. As a group and individually these survivors were meeting public physical activity guidelines and engaging in the recommended weekly minutes. The high sitting time coupled with the high overweight and obesity levels highlight the need for positive behavioural changes including improved levels of physical activity and reduced sedentary behaviour. These changes need involvement from the numerous levels of society that affect health. Broad physical activity guidelines need to be developed not only to improve physical activity levels in breast cancer survivors but to work as a preventative measure by facilitating physical activity promotion in the general population. Get sleep or get stumped: sleep behaviour in elite South African cricket players during competition

- Description: Introduction: Good sleep behaviour is associated with achieving optimal athletic performance and reducing the risk of injury. Elite cricket players have unique physical and cognitive demands, and must accommodate for congested competition and travel schedules (all of which increase the risk of disruptive sleep). Further, the political pressures and socioeconomic barriers in South African cricket could affect the sleep of the country’s elite players. Previous research in cricket has focussed on the impact that nutrition, equipment specifications, movement physiology and psychology could elicit on performance (where many professional teams hire support staff to supervise these disciplines); however, there is limited empirical application of sleep research in elite cricket players. Therefore, this study aimed to characterise the sleep behaviours of elite South African cricket players during periods of competition and investigate the relationship between pre-match sleep and cricket performance. Methods: A longitudinal field-based investigation was implemented to monitor the sleep behaviour of 26 elite South African cricket players (age: 28.6 ± 4.0 years; height: 1.8 ± 0.1 m; weight: 85.7 ± 10.8 kg; elite experience: 3.7 ± 4.0 years) during home and away competitive tours. The Morningness-Eveningness Questionnaire and Athlete Sleep Behaviour Questionnaire were administered to identify chronotype and poor sleep behaviours. Players completed an altered version of the Core Consensus Sleep Diary every morning post-travel, pre-match and post-match. Linear mixed model regression was used to compare differences in sleep variables between time-periods, match venues, player roles, match formats, sleep medication and racial groups. Spearman’s correlation (rs) was used to assess the relationship of substance use (alcohol and caffeine), age, elite experience and match performance with selected sleep indices. Statistical significance for all measures was accepted at p < 0.05. Hedge’s (g) were used as the measure of effect size. Results: Light-emitting technology use, effects of travel, late evening alcohol consumption and muscle soreness were the main factors that impacted sleep. Post-match total sleep time (06:31 ± 01:09) was significantly (p < 0.05) shorter compared to post-travel (07:53 ± 01:07; g = 1.19 [0.81;1.57]) and pre-match (08:43 ± 01:03; g = 1.97 [1.55;2.39]) total sleep time. Post-travel sleep onset latency and sleep efficiency were significantly (p < 0.05) shorter (g = 0.74 [0.29;1.29]) and higher (g = 1.35 [0.76;1.94]) at home than away. Although not significant (p > 0.05), allrounders took longer to fall asleep (g = 0.90 [0.23;1.57]), obtained less total sleep (0.76 [0.29;1.42]) and had lower morning freshness scores (g = 1.10 [0.42;1.78]) the night before a match compared to batsmen. Wake after sleep onset and get up time were moderately longer (g = 0.61 [0.22;1.26]) and later (g = 0.62 [0.27;1.17]) before. Twenty20 matches compared to One-Day International matches respectively. Further, sleep duration significantly declined from pre-match to post-match during the multi-day Test format (p = 0.04, g= 0.75 [0.40;1.12]). Late alcohol consumption was significantly (p < 0.05) correlated with a decrease in total sleep time, regardless of match venue (home: rs (49) = -0.69; away: rs (27) = -0.57). During the away condition, an increase in age was significantly associated with longer wake after sleep onset durations (rs (13) = 0.52, p = 0.0003), while greater elite experience was significantly associated with longer total sleep time (rs (72) = 0.36, p = 0.02). The non-sleep medication group took significantly longer to fall asleep compared to the sleep medication group during the first week of the away condition (p = 0.02, g = 0.75 [0.25;1.26]) particularly on nights following transmeridian travel. Although not significant ( p > 0.05), Asian/Indian players had moderately longer sleep onset latencies (g = 1.07 [0.66;1.47]), wake after sleep onset durations (g = 0.86 [0.42;1.29]), and lower subjective sleep quality (g = 0.86 [0.46;1.26]) and morning freshness scores (g = 0.89 [0.47;1.27]) compared to Whites. Similarly, Black Africans had moderately lower subjective sleep quality scores compared to Whites (g = 0.71 [0.43;0.97]). Longer sleep onset latencies and shorter total sleep times were significantly (p < 0.05) associated with poorer One-Day International (rs (28) = -0.57) and Test (rs (12) = 0.59) batting performances respectively. Higher subjective sleep quality scores were significantly associated with better Twenty20 bowling economies (rs (8) = -0.52). Discussion: There was no evidence of poor pre-match sleep behaviour, irrespective of venue; however, the most apparent disruption to sleep occurred post-match (similar to that found in other team-sports). Most disparities in sleep between match venues existed post-travel, with better sleep behaviour observed during the home condition. The differences in sleep patterns found in all three match formats were expected given the variations in format scheduling and duration. Although sleep medication was shown to promote better sleep, its long-term effectiveness was limited. The results promote the implementation of practical strategies aimed to reduce bedtime light-emitting technology use, late evening alcohol consumption and muscle pain. Inter-individual sleep behaviour was found between player roles, age, experience level and race. Moderate associations existed between sleep and markers of batting performance, specifically for the longer, strategic formats of the game. Conclusion: The current study provided new insight of the sleep behaviour in elite South African cricket players during competition. Individualized sleep monitoring practices are encouraged, with specific supervision over older, less experienced players as well as the racial minorities and allrounders of the team. The poor post-match sleep behaviour, together with the sleep and performance correlations, provide ideal opportunities for future interventions to focus on match recovery and the use sleep monitoring as a competitive advantage.

Quantification of pre-competitive sleep/wake behaviour in a sample of South African cyclists

- Description: The quantification of athlete pre-competitive sleep behaviour is of interest owing to the possibility that sleep loss may have a negative effect on health and performance. The purpose of this study was to monitor and quantify the sleep/wake patterns of South African cyclists prior to competitive races. A total of 336 cyclists, male and female and of differing competition levels, cycling in either the 2015 Tsogo Sun Amashova or the 2016 Telkom 94.7 Cycle Challenge completed an altered version of the Competitive Sports and Sleep Questionnaire. The questionnaire asked cyclists to report on precompetitive sleep over the past year. A subset of 92 cyclists also recorded a Core Consensus Sleep Diary for the three nights leading up to the races. The questionnaire showed that 67% of the cyclists reported worsened sleep at least once prior to competition within the past 12 months. The sleep diary found that the cyclists’ average sleep duration the night before the races was 6h19min (±1h38min), which was significantly less than two and three nights prior to the races. Sleep quality was also shown to deteriorate significantly the night before the races. The contributing factors leading to worsened pre-competitive sleep were the time the cyclists had to wake-up as well as perceived increases in sleep latency and awakenings after sleep onset. Anxiety was found to be the major cause of sleep disturbances. While females were found to be significantly more likely to report having experienced poorer sleep before competition in the past year, the sleep diary showed no difference in sleep the night before the races between the sexes. Females were significantly more likely to report instances of unpleasant dreams and waking up during the night. Again, the sleep diary data did not corroborate these findings. Females were also found to report significantly more accounts of nervousness or thoughts about competition as being the cause of sleep problems. There was no difference in sleep loss the night before competition when comparing competition-level groups. The only significant difference was that recreational cyclists were more likely to report sleeping in foreign environments as a cause of sleep disturbances. Despite a large percentage of cyclists experiencing pre-competitive sleep loss, over half (55%) perceived sleep loss to have no impact on their performance. Analysis of pre-sleep behaviour also revealed that the cyclists engaged in several practices that may have a negative effect on subsequent sleep. The vast majority of the cyclists (61%) indicated having no specific strategy to help them sleep the night before competition. Fifteen percent of cyclists reporting using media devices to help them fall asleep, a practice that has been shown to disrupt sleep. In conclusion, most cyclists, regardless of sex and level of competition experience precompetitive sleep loss attributed largely to anxiety but with the perception that this loss in sleep does not negatively impact their performance.

Strength and conditioning practices of high school rugby coaches: a South African context

- Description: Although the sport of rugby union is well established, the strength and conditioning practices of high school level players are not well known. Therefore, the purpose of this study was to examine the current strength and conditioning practices that coaches implement at South African high school level rugby. A secondary purpose was to compare practices between different types of schooling systems available in South Africa. An online survey or in person interview (depending on the school), adapted from previous strength and conditioning questionnaires, was conducted with 43 responses; including 28 schools among the top 100 rugby schools in South Africa for 2016 and 15 no-fee paying public schools in the Eastern Cape Province of South Africa. Results indicated that the top 100 rugby schools implement conditioning practices similar to the best-known international practices compared to no-fee paying schools who lacked the knowledge and skills in various strength and conditioning principles. It was found that all no-fee paying school coaches had insufficient qualifications to administer the correct training techniques. Coaches at all schools lacked the appropriate knowledge on injury prevention and scientifically based training programmes. It was concluded that education and skills around the best strength and conditioning practices for school level coaches needs to be improved and particularly in less privileged schools. The main goal being to reduce the risk of injury and improve performance across all sectors of the rugby playing population within the country. This was deemed crucial to the transformation goals set out by the South African Rugby Union, which would benefit from player development in lower socioeconomic schools.

Selected physiological and perceptual responses of batsmen during a simulated one day international century : impact on performance

- Description: Background: Very few studies have examined the demands of prolonged batting on physical, psychophysical and performance responses. Objective: The purpose of this study was therefore to determine the physiological and perceptual responses of batsmen scoring a simulated One Day International century. Furthermore, the impact that these responses had on batting performance were also measured. Methods: Seventeen male cricketers currently playing for Rhodes University or the country districts side, performed a simulated bating innings (BATEX©), typical of scoring a One Day international century. The BATEX© protocol consisted of six, five over stages (21 minutes each), with each stage matched to a specific phase of play where batsmen were encouraged to bat with the mindset of that phase. During stages one, three and five players ran at a “self-selected cruise pace” whereas during stages two, four and six players were required to run at maximum speed. At set intervals, selected physiological, perceptual and performance measures were recorded. Results: Most physiological responses increased and decreased accordingly with the low and high intensity stages with the exception of respiratory exchange ratio and core temperature responses. Mean physiological responses however, showed a progressive rise over time. Central ratings of perceived exertion increased as a function of intensity and duration. Sprint times got slower as the protocol progressed, while accuracy (impact accuracy) improved significantly (p<0.05) after stage one and then stabilised for the remainder of the protocol. Conclusion: The simulated batting protocol significantly impacted the physiological and perceptual responses over time. This was due to both the intensity and the duration of the work-bout. Furthermore, the increased physiological load placed on the batsmen significantly impacted the physical performance of the batsmen. An increase in sprint times (players getting slower) towards the end of the protocol may indicate the presence of physical fatigue. This physical fatigue did not however seem to impact the batsmen cognitively as accuracy improved after the first stage and then remained constant over time.
