- Description: Background: Despite the global aviation industry's ongoing commitment to safety, crew fatigue remains a persistent and pressing issue. Within South Africa, fatigue management was primarily governed by regulations, however, there was ongoing debate about whether these regulations adequately reflected the latest scientific evidence and operational demands. This study addressed this gap by investigating perceptions about fatigue and fatigue management from management and decision-makers (blunt-end stakeholders) within the South African aviation sector. This study aimed to investigate the perceptions surrounding fatigue and fatigue management among management and decision-makers (blunt-end stakeholders) within the South African aviation sector. Specifically, it sought to contribute to ongoing research dedicated to developing a better understanding of whether, how, and why regulations pertaining to fatigue management may need to change. By exploring the perspectives of blunt-end stakeholders, the study aimed to uncover insights into the challenges and potential gaps in current regulations, with the ultimate goal of informing potential regulatory adjustments to enhance aviation safety. Methodology: A cross-sectional, qualitative exploratory design investigated perceptions about fatigue and fatigue management among blunt-end stakeholders in the South African aviation industry. Data was collected through online semi-structured interviews and written open-ended questionnaires to capture diverse perspectives. The methodology aimed to comprehensively understand fatigue-related experiences and perceptions among pilots and cabin crew, considering the unique challenges posed by the COVID-19 pandemic. Participants were selected using purposive and snowball sampling, ensuring representation from various regulatory categories and industry sectors. The study employed a thematic analysis approach to analyse data collected through online semi-structured interviews and written open-ended questionnaires. Thematic analysis facilitated the identification of key patterns and themes in the perceptions of blunt-end stakeholders within the South African aviation industry regarding fatigue and fatigue management Results: The thematic analysis of data from thirteen South African aviation industry stakeholders unveiled insights into crew fatigue challenges and regulatory concerns. Participants emphasized crew fatigue as a significant safety risk linked to entrenched crew mentalities, where instances of disregarding fatigue management protocols posed imminent safety risks. Additionally, outdated regulations were cited, hindering optimal crew rest periods and modern fatigue mitigation technologies, with discrepancies in regulatory support exacerbating challenges across operational contexts. Aviation stakeholders expressed apprehensions regarding the efficacy of existing Flight and Duty Regulations, advocating for amendments to enhance fatigue management practices. The COVID-19 pandemic further exacerbated fatigue issues, leading to shifts in crew morale, increased stress levels, and regulatory challenges, necessitating adaptive fatigue management strategies and proposed risk mitigation plans, including regulatory adjustments and enhanced stakeholder engagement. Discussion: The study investigated the intricate landscape of fatigue-related risks within the aviation industry, recognizing the systemic and context-specific nature of these challenges. It is evident from our findings, in line with previous research, that fatigue within the aviation sector is not merely an isolated issue but a multifaceted consequence of systemic factors. Our study highlights the pressing need for revisiting the current Flight and Duty Periods (FDPs) and overall fatigue management strategies to address these underlying systemic issues effectively. Unlike static safety concerns, fatigue-related risks evolve dynamically, influenced by technological advancements, industry practices, and external factors such as the ongoing COVID-19 pandemic. Our analysis elucidated the complex interplay between safety protocols, operational standards, and the well-being of crew members, emphasizing the need for a nuanced approach in mitigating fatigue-related risks. Central to our findings is the recognition of the limitations of existing regulations in adequately addressing emerging challenges associated with crew fatigue. Our study underscores the imperative of regulatory agility in safeguarding both passengers and crew members, advocating for continuous evaluation and refinement of regulatory frameworks. Specifically, there is a critical need to revisit and adapt current FDPs and fatigue management strategies to align with the systemic causes of fatigue identified in our research. Conclusion: This study contributed a comprehensive analysis of crew fatigue in South African aviation, focusing on perspectives of blunt-end stakeholders. It highlighted widespread recognition of fatigue's safety implications, exacerbated by factors like economic stressors and regulatory deficiencies, including those intensified by the COVID-19 pandemic. Participants advocated for regulatory reforms to enhance crew rest provisions and address scheduling challenges. The research underscored the need for a proactive, collaborative approach prioritizing aviation professionals' well-being. - Description: Intro: Accidents are complex in nature with multiple contributing factors. The way in which accidents are investigated is important and using system-based analysis tools assists in understanding and mapping these contributing factors to learn from them. There has been an increase in the number of accidents that have occurred within the general aviation industry in South Africa and while accident investigations have been undertaken, these have not included the application of system-based analysis tools. This led to a collaboration between Rhodes University and the Accident and Incident Investigations Division (AIID) of the South African Civil Aviation Authority where it was agreed that two systems-based analysis tools will be applied to a previously investigated accident that occurred in 2015. Aims: The first aim of this thesis was to identify if, through the implementation of these systems-based tools, the systemic contributory factors could be determined using the existing report by the AIID. The second aim of this thesis was to identify if, using the two systems-based tools, the actors and levels involved in the accident could be identified and the third aim was to identify if the implementation of these tools generates the same or different recommendations to that of the AIID. Methods: The two systems-based analysis tools applied were AcciMap and Causal Analysis using Systems Theory (CAST). These tools were applied to the V5-NRS Cessna 441 Conquest II accident report which captured the details of how the aircraft flew into the Tygerberg mountain on its descent into the Cape Town International Airport in August 2015. Results: Through the application of these two systems-based analysis tools the major contributing factors elucidated throughout this analysis were: visual and lighting conditions, pilot experience, training, lack of terrain warning equipment, fatigue, inadequate oversight, and inadequate risk management. In line with these findings, the analysis revealed various actors across various levels (the crew; South African Air Traffic Control, the SACAA, WestAir (the operator) and the Namibian Civil Aviation Authority Through the elucidation of these factors at various levels, 14 to 15 different recommendations were generated which was more than the one recommendation that was generated by the AIID. Discussion: Even when applied to an existing report, both the CAST and Accimap tools were able to bring to light the systemic contributing factors to this accident and importantly, highlight the role that various actors and levels within the system had in this unfortunate event. Consistent with previous literature, most of the contributing factors were found at the lowest level (the crew in this case) and fewer, but key factors were identified at higher levels (management and regulator level). Importantly, the application of the systems tools facilitated a systematic and systemic analysis of this accident, which allowed for the generation of recommendations at all levels, not just at the operator level. Conclusion: This study demonstrates the benefits and importance behind implementing a systems-based analysis method to an accident as these tools generate more useful recommendations which allows for important lessons to be learned following accidents, with the intention of re-designing systems to prevent them from happening again. , Thesis (MSc) -- Faculty of Science, Human Kinetics and Ergonomics, 2023

The effects of different shift patterns on nurses’ sleep-wake behaviours in selected, private healthcare facilities

- Description: Nurses are required to work shifts to provide 24-hour care, in which they complete physically and mentally demanding tasks. The length and type of shifts, particularly night shifts interfere with the natural sleep-wake behaviours, leading to extended wakefulness and overall reduced sleep, and increase the likelihood of sleepiness during subsequent shifts. This can in turn affected various cognitive processes such attention, vigilance and alertness, which are necessary during the care process. Sleepiness as a result of working shifts has also been associated with an increased risk accidents and error during the delivery of care. Given the unique demands and ways in which workplaces are structured, each context arranges its shifts in unique ways and thus, in order to determine how to manage the effects of shift work, it is important to understand how it affects self-reported fatigue and sleep, of, in this case, nurses. While there has been extensive research on this in the global north, to date, there has been limited research aimed at examining the effects of shift work on nurses’ sleep-wake behaviours and fatigue in the South African context. Therefore, the aim of this study is to characterise shift arrangements in selected private facilities and explore its effects on private healthcare nurses. This study adopted a cross-sectional, survey design using an amended version of Standard Shiftwork Index. The questionnaire included demographic and shift details and explored the impact of the shift systems on nurse sleep-wake behaviours and disturbances and fatigue and workload. It was distributed among shift working nurses registered with the South African Nursing Council across three selected, private, healthcare facilities in the Eastern Cape, over a two-month period. The responses were analysed with descriptive and inferential statistics, with open-ended questions analysed using a thematic analysis. A total of 51 nurses completed the survey. Nurses worked 12-hour shifts which included night shifts and day shifts with fixed start and end times. Over time was commonly reported and generally, nurses reported having very little control over their shift arrangements. Overall, nurses slept less than what they reported they needed on duty days, with nurses working both day and night shifts reporting to sleep less than the globally recommended required sleep. This was compensated for by longer sleep durations during days off. The data collection revealed that three different shift arrangements were in use, including permanent day shifts, permanent night shifts and rotating shift work including nights, with permanent night nurses working significantly more consecutive shifts (seven) than the other two shift types and having significantly more days off (seven) as well. While there were no significant differences in self-reported sleep across the three shift types, permanent night nurses were found to have the shortest sleep. During days off, rotating nurse reported significantly longer sleep times compared to day shift workers which may point to the need to catch up from sleep debt. Rotating nurses experienced the greater total disturbances to their sleep than permanent day and permanent night shift nurses. While not statistically significant, it may point to the fact that rotating shift workers could not obtain regularly timed sleep (due to having to change their schedules) compared to permanent day and night nurses. Workload (physical, emotional, mental and time pressure) did not differ between the shifts (day or night) or the shift types, but did reflect a heavier workload, possibly due to the data collection occurring during the 5th wave of the COVID 19 pandemic. This study highlights that nurses in private healthcare facilities are working extended hours which were associated with reduced total sleep, irrespective of the nature of the shift, with rotating shift nurses experiencing some degree of greater disturbances to their sleep. The number, duration speed and direction of the shifts of rotating nurses needs to be explored further, whilst also exploring the influence of individual factors on sleep-wake behaviours of nurses. Characterising the sleep wake behaviour of late adolescents living in rural and township areas surrounding Alice in the Eastern Cape of South Africa

- Description: Introduction: School going adolescents have been identified as a group that is at risk of obtaining insufficient sleep due to shortened and irregular sleep, all of which can affect the overall wellbeing of adolescents. While there has been extenstive research on adolescent sleep around the world, research in South Africa has been limited. Of the research performed in this context, most has been conducted in city or urban contexts, but there has been comparatively less research on sleep in adolescents from rural and township areas. Therefore, this study focused on characterising and comparing the sleep-wake behaviour of late adolescents from selected rural and township areas in Alice in the Eastern Cape of South Africa. Methods: An adapted version School Sleep Habits Survey (SSHS) was included in the study and assessed demographic, academic, sleep-wake behaviour, lifestyle, and behavioural information of adolescents from rural and township schools in and around Alice. The survey was distributed physically to all the learners from four co-educational rural and township schools. The responses to the survey were analysed with descriptive and non-parametric inferential statistics, as all data were not normally distributed. Results: A total of 123 learners completed the survey. All the participants were in Grade 12, aged 16 to 26 years (median =18, IQR= 18-19) from rural (n=28) and township (n=95) schools. Seventy-four female and 49 male learners participated in the study. Within the sample, there were also a group of participants who were not adolescents (n=22) whose age ranged between 20 and 26. Reported sleep during both the week and weekend (7hours) was lower than both the recommended duration for adolescents and their own self-reported sleep need of a median of 8 hours and 14 minutes. Bedtimes were significantly earlier on weekdays than weekends. The need to do homework and feeling sleepy were cited as the main reasons for going to sleep during the week and the weekends. Wake times on weekdays and weekends did not differ, with most learners citing being woken up by an alarm clock as the main reason for waking during both weekdays and weekends. Most learners (n=47; 38%) reported walking to get to school, while 41 (33%) relied on public transport to get to school. There were no significant effects of gender or of being rural and township scholars in relation to their sleep variables. Comparisons between non-adolescents and adolescents revealed that non-adolescents reported sleeping longer (7 hours and 30 minutes) compared to adolescents (7 hours) than adolescents did on weekends. As a group, the learners in this sample were morningness-orientated in terms of chronotype, presented with low day time sleepiness, some evidence of sleep-wake behaviour problems and of depressive mood symptoms. Nearly half (n=53; 43%) the group reported napping occasionally on school days. In terms of caffeine consumption, just over half (n=64; 52%) of the learners reported never consuming caffeinated substances such as soda, coffee, or tea. Conclusions: The reported sleep duration of learners in this study was consistent on both the week and weekend but was shorter than the recommended 8-10 hours for late adolescents. While learners in this sample reported early bedtimes, sleep was likely curtailed by the early rise times due to the contextual constraints where scholars had to commute, by either walking or taking public transport, to make the early school start times. There was a high prevalence of sleep disturbances in this sample however, the effects of this (increased daytime sleepiness) were lower which can be attributed to low reported prevalence of caffeine consumption combined with the high prevalence of reported day time napping on school days and weekends. While only a snapshot, the results of this study support previous research that has highlighted the challenges that many adolescents across the globe experience when it comes to obtaining enough sleep. The results of this study highlighted the need for more research to understand the sleep-wake behaviours of learners and their lifestyles more objectively. Given the early starts for many participants in this study, interventions such as delaying school start times may help to improve the amount of sleep that learners get during the week. Drifting towards death: a South African patient safety incident through an HFE Systems lens

- Description: Patient Safety Incidents (PSI) are a frequent occurrence within the South African public healthcare system wherein a patient is unnecessarily maimed, harmed, killed, or put through significant trauma, emotional or physical. These incidents have a significant impact on the performance of the system and the well-being of individuals involved. Often PSI are the result of multiple system failings that provide the necessary preconditions for the PSI to occur. Thus, to provide appropriate patient safety recommendations to address and aid in the prevention of future PSI it is necessary to apply a systems approach to PSI analysis. A systems approach supports a ‘bigger picture’ view of an incident which includes looking beyond the immediate causes of a PSI and taking the different levels of the healthcare system into consideration during incident analysis. Human Factors and Ergonomics (HFE) is at its core a systems discipline and has been successfully applied to multiple fields including healthcare. HFE offers multiple incident analysis tools grounded in systems theory. The Life Esidimeni incident, a PSI that resulted in the death of 144 MHCU, is the biggest PSI in recent South African history and is therefore an important potential case study for the application of HFE systems tools within the South African healthcare context (an area that is lacking in existing literature). The objectives of this research were to (i) Systematically uncover the causal factors that led to the outcome of the of the Life Esidimeni incident; (ii) Identify critical faults, and gaps within the healthcare system that led to the Life Esidimeni PSI; and (iii) Provide proactive recommendations for future prevention of PSI. To fulfil these objectives a descriptive case study research method design was adopted using a qualitative systems-based tool, AcciMap. The application of AcciMap to Life Esidimeni enabled both the sharp end and blunt end causal factors that contributed to the outcome of the incident to be identified. Importantly this provided insight into the critical faults and gaps of the South African public healthcare system. The results of the AcciMap indicated that there were four main broad systemic faults in the system. These broad areas were categorized as key themes, which include: (i) competency, (ii) safeguards, (iii) time pressures, and (iv) vertical integration. - Description: Fatigue in aviation results from the complex interaction of various factors (both work and non-work-related) that are important to understand when attempting to manage it. Managing fatigue effectively is important given that it has and continues to influence crew wellness and aviation safety. One of the ways of managing fatigue is through appropriately designed Flight Time Limitations (FTLs) however; in South Africa there have been calls from the unions and the crew to update these in line with the latest science and operational demands and insights. Doing this requires the generation of context specific data, which this thesis aimed to provide as an initial step. Firstly, this study aimed to explore the reported prevalence of fatigue across the South African aviation industry. Secondly, it elucidated what factors (both work and non-work related) crew perceived contributed to fatigue and lastly, the crew‟s perceptions regarding the current FTLs were explored. Methods: To realize the aims of the study, an online survey was developed using existing literature initially, and through consulting with and getting information from aviation industry stakeholders and other experts in the field of aviation and fatigue. This consultation (which occurred over a number of iterations of the survey) ensured that there was a balance between the scientific and the actual operational perspectives on how best to explore crew perceptions around fatigue. The finalized survey was divided into five parts: part one was consent to participate; part two included questions pertaining to the participants‟ demographic information; part three included questions pertaining to the crews‟ perceptions around fatigue (its prevalence, its impact on safety and how it affected crew); part four explored crew‟s perceptions on fatigue contributory factors, both work and non-work related; finally part five included questions pertaining to crew perceptions about the current South African FTLs, specifically their concerns, suggestions for improvement and any aspects they considered as fatigue mitigating. The survey was made available via the South African Civil Aviation Authority‟s (SACAA) website as well as via the industry stakeholder‟s networks that encouraged all crew across the different sectors of the industry to participate. All numerical data collected were analysed descriptively through inferential statistics, while the qualitative data were analysed using thematic analysis. Results: 194 participants completed the survey, 167 were from cockpit and 27 were from the cabin crew. The results of the study highlight that there is a high perceived prevalence of fatigue. The crew also recognized that fatigue is a significant safety risk, but less indicated that fatigue interfered with their ability to do their duties. Crew reported that the length of duties, number of sectors flown, insufficient sleep at night, early sign-on‟s, late sign-offs, working too many consecutive days in a row, inadequate or irregular sleep before and during periods of duty, night flying, bad weather, severe turbulence, having young children or dependents to look after, financial stress, extended commuting to get to and from work, and poor diet were some of the work and non-work-related contributory factors to fatigue. In line with these findings, questions around the perceived concerns about the current FTLs revealed that crew were concerned about unclear definitions of the civil aviation regulations (CARs), the lack of control of disruptive rostering schedules, periods of high workload due to the number of sectors flown per duty, the length of duty periods and effects of being on standby duty, inadequate rest between duties and strings of duty. The crew also had concerns regarding the Flight Duty Periods (FDPs) where the crew were concerned with the flying limits being used as targets by the operators, a lack of science applied to the FDPs, and that the current FDPs are outdated. The recommendations included limiting disruptive rostering schedules by altering standby provisions for the crew, instilling a block roster schedule, disallowing double signing on and off on the same day, and reducing split shifts. The crew also recommended adjusting duty durations by adjusting daily and monthly limits, tapering duty lengths, but also limiting discretionary extensions. Increasing rest provision was another recommendation suggested by the crew and included increasing the number of rest days off as well as the minimum hours of rest between duties needs to be increased. The fatigue mitigating aspects were minimal if any. Conclusion: Overall the study revealed that there is a high perceived prevalence of fatigue within this sample of the South African aviation industry and that the concerns outlined by crew around the contributory factors to fatigue are consistent with previous research, but also reflect the unique operating context of South Africa. This study serves as a base from which to explore more specific areas of the crew working time that are disruptive to sleep. This may help operator‟s roster duties in a more predictable way to limit the incidence of fatigue, while also offering the opportunity for the regulator and other stakeholders to focus their efforts on how to better design the current FTLs to limit the prevalence and risks associated with crew fatigue. , Thesis (MSc) -- Human Kinetics and Ergonomics, 2022

A laboratory simulation investigating the impact of sunglass tint on the catching performance of cricket fielders

- Description: Background: Sunglasses are popularly worn by outdoor athletes such as cricketers. They are primarily worn to preserve ocular health due to the length of time exposed to the sun’s glare on the field. More recently, sunglasses are now worn for their purported performance-enhancing benefits supposedly allowing athletes to “see clearly, react faster and perform with confidence”. Fielders typically wear sunglasses, but it is rare to see the bowler or the batter wearing sunglasses although they are exposed to the same conditions. If sunglasses provide visual clarity for better reaction time, it is assumed this would also be beneficial for both batsmen and bowlers alike. It is interesting that although fielding plays a key role in the winning of a match, it has received very little research in comparison to the areas of batting and bowling. Research concerning sunglasses in the past has been concerned with its effect on ocular parameters such as colour vision and contrast sensitivity, with minimal research on their effect on performance, specifically on how different colour tints may affect performance such as catching in fielding. Objective: The purpose of this study was to compare the catching performance of fielders when wearing three different colour tints of sunglasses. Methods: Thirty male cricketers currently playing for Rhodes University or country districts sides in Makhanda, South Africa, who were non-habitual sunglass wearers were recruited for this study. An optician administered a pre-screening test to ensure participants did not have any visual defects that would affect the results, determining who was eligible to continue with the research project. Twenty-five participants qualified with a further four withdrawing during the experimentation phase due to personal reasons. Eligible participants were required to complete four testing sessions on separate days thereafter in which they wore a different colour tint at each session (clear, blue, G30 and red). These tints were randomised among participants. Participants were required to catch 18 balls projected from a bowling machine in each session. The number of balls caught as well as the quality of catch was recorded. At the end of the fourth session, participants were presented with a questionnaire investigating their subjective experience of the protocol and preference for any particular colour of lens. Results: Statistically significant results were found between the different colour lenses for the ocular parameters of contrast sensitivity and stereopsis tested by the optician. The red lens was found to be the best for contrast sensitivity with the blue lens performing the worst on this test. With regard to stereopsis, the G30 lens was the worst, with the red lens performing the best. The performance measures resulted in no significant difference between the different colour lenses both concerning the total number of catches taken as well as the quality of catch taken. The same result was found irrespective of the colour of lens used in each session. Data gathered from the questionnaire regarding visual comfort and target visibility resulted in the red lens being rated statistically worse than the other three conditions. In terms of personal preference, the G30 lens was rated statistically as the best coloured lens. Conclusion: It can be concluded that objectively there is no difference in performance between the three coloured lenses. Although from an ocular perspective, the red lens was expected to perform the best in terms of catching performance, there was no difference. - Description: Working in academe is characterized by high work demands, long and late working hours and balancing multiple roles. This often leaves academic staff (interchangeably referred to as ‘academics’ and ‘staff’) with limited time to prioritize adequate rest and sleep. While sufficient sleep is important for general well-being and work performance, it may be important for academics given the cognitive and interactive nature of their work. However, limited research has characterized sleep behaviours and sleep quality in academic staff globally and specifically in South Africa (SA). Further, research shows that academics from different academic ranks and genders differ in their work experiences but less is known about whether these factors affect their sleep and lifestyle factors. Over and above the demands of academic work, the current study was conducted during COVID-19, which saw unprecedented change in workloads for academic staff due to online teaching and learning. Additionally, the pandemic has been associated with poorer sleep quality and lifestyle behaviours in the general population, but very few studies have explored this in the context of academics. Thus, this study aimed to characterize the sleep quality, work and lifestyle factors among academics from a university in the Eastern Cape province of SA and determine the effects of gender and academic rank on these measures. Additionally, the study aimed to determine the impact of COVID-19 on sleep and work-related factors of this group. This study adopted a cross-sectional design and made use of an online questionnaire that was distributed to all academics employed at the institution via email. The data collection occurred over an 8-week testing period between August and September of 2020. The questionnaire explored sociodemographic, work and lifestyle characteristics, while the characteristics of sleep and sleep quality were explored with the Pittsburgh Sleep Quality Index (PSQI). Additionally, an open-ended section was included to determine the impact of COVID-19-related changes on sleep and work. The questionnaire responses were analyzed with descriptive and inferential statistics and the open-ended questions were analyzed with thematic analyses. A total of 84 respondents completed the questionnaire. On average, sleep quality was poor (global score of 7.09 ± 3.8), the reported sleep duration was short (6.41 ± 1.06 hrs) and the group was classified as overweight according to median BMI (26.67 [23.8-2 29.45] kg/m2). Over a third (35.71%) of academic staff reported not meeting physical activity guidelines and reported using screens one hour prior to bed every night during the week (76.19%) and the weekend (67.88%). Staff also worked long total weekly work hours (54.25 ± 11). Female academics had a poorer sleep quality (p = 0.035), spent a longer time in bed (p = 0.04), experienced more bad dreams (p = <0.01), had their last serving of alcohol earlier (p = 0.04) and worked less weekday (p = 0.04) and less total weekly (p = 0.02) hours compared to male academics. Professorship-level academics were significantly older than junior- and senior-level academics (p = <0.01) while senior-level academics had a poorer PSQI compared to professorship-level academics (p = 0.03). The thematic analysis showed that home environment, neighbourhood noise, work and mental health were the main emerging factors that disturbed sleep. The thematic analysis showed that, in relation to the impact of COVID19, sleep schedules changed and sleep duration and sleep quality improved or got worse. Respondents reported their work hours increasing, work schedules becoming less routine while working from home, dealing with the challenges of working online and experiencing worry, stress and anxiety due to the pandemic. The study highlights that, in the context of lockdown and having to work and teach online from home, academics reported poor quality sleep and short sleep duration. Further, on average, the sample reported working extensive hours and unhealthy lifestyle behaviours. The sleep health, nature of work and overall lifestyles of academic staff deserves more research attention, given the importance of their work and the impact that inadequate sleep could have on academics’ health and work performance. Sleep quality was worse for females and mid-career academics, suggesting that their sleep quality may be at greater risk than other sub-groups. However, the reasons for these differences need to be explored in future studies. The findings contribute to the narrative that academe involves numerous demands and supports previous research that has suggested that academics’ sleep is insufficient. Furthermore, COVID-19 had an impact on staff’s sleep, with staff reporting changes in their sleep quality and duration. The pandemic impacted work of academics by presenting novel demands related to online teaching, intensifying demands overall and disrupting work routines. In low-to middle-income (LMIC) countries, such as South Africa (SA), limited financial, material and human resources coupled withpoor infrastructure and poor public health determinants, includingpoverty andpoor education, affectthe ability to maintain andimprove on quality care outcomes.Understanding what different stakeholders perceive as barriers, and if these barriers are understood at different levels, is therefore important when attempting to mitigate the risk for unsafe or inefficient care delivery. Human Factors and Ergonomics (HFE) adoptssystems and participatory approaches for the exploration, analysis, and design of socio-technical systems to optimize both human wellbeing and system performance.The barriers to safe and effective healthcare delivery, from an HFE perspective, are not known in the South African context, particularly in parts of the Eastern Cape Province. Elucidatingthesebarriers, even if self-reported,may guidefuture efforts aimed at mitigating risks.The purpose of this study, therefore,wasto explore and highlight the perceived systemic barriers to local and national healthcare delivery, within the Sarah Baartman District in the Eastern Cape Province of South Africa.Methods: Ashort discussion aimed at introducing HFE and components of the Work Systems Model, followed by a survey that captured participant demographics, job characteristics, the perceived national and local systemic barriers, and proposed solutions, was administered withhealthcare stakeholders from 14 primary healthcare facilities and 1 department office within the Sarah Baartman District.Participants (n=120) included management, pharmacy, administration, maintenance, community-and home-based care and nursing staff.Data from the surveys were thematically analysed and categorised according to components of the work system model (Carayon, 2009) and respective workgroup.Results: The findings revealed many overlapping,systemic barriersthat includedshortages of staff, poor management and leadership, a lack of equipmentand basic necessities, poor infrastructure, patient complexity,and high workloads. The results further indicate that the way in which the reported barriers affect worksystem interactionsand performance are unique to different workgroups. Stakeholders iiproposedthat,among others, the absorption of contract workers, the provision of training and adequate human and medical resources and the maintenance of facilities may mitigate the barriers and improve healthcare delivery.Conclusion: The findings highlight a myriad of perceived systemic barriers perceived in the Sarah Baartman district, some of which were fundamental for the effective function of any healthcare system. These barriers may have wide-spread implications for stakeholders at all levels, ultimately affecting the performance, satisfaction and safety and the quality of care. It is especially important to consider these barriers in light of the COVID-19 epidemic, which emerged throughout this study and the major threat it presents to South African healthcare systems. Future research should aim to explore how these barriers interact to contribute to processes and outcomes, as well as explore the perceptions at provincial and national levels in order to better identify areas and strategies for improvement.

- Description: Introduction: Good sleep behaviour is associated with achieving optimal athletic performance and reducing the risk of injury. Elite cricket players have unique physical and cognitive demands, and must accommodate for congested competition and travel schedules (all of which increase the risk of disruptive sleep). Further, the political pressures and socioeconomic barriers in South African cricket could affect the sleep of the country’s elite players. Previous research in cricket has focussed on the impact that nutrition, equipment specifications, movement physiology and psychology could elicit on performance (where many professional teams hire support staff to supervise these disciplines); however, there is limited empirical application of sleep research in elite cricket players. Therefore, this study aimed to characterise the sleep behaviours of elite South African cricket players during periods of competition and investigate the relationship between pre-match sleep and cricket performance. Methods: A longitudinal field-based investigation was implemented to monitor the sleep behaviour of 26 elite South African cricket players (age: 28.6 ± 4.0 years; height: 1.8 ± 0.1 m; weight: 85.7 ± 10.8 kg; elite experience: 3.7 ± 4.0 years) during home and away competitive tours. The Morningness-Eveningness Questionnaire and Athlete Sleep Behaviour Questionnaire were administered to identify chronotype and poor sleep behaviours. Players completed an altered version of the Core Consensus Sleep Diary every morning post-travel, pre-match and post-match. Linear mixed model regression was used to compare differences in sleep variables between time-periods, match venues, player roles, match formats, sleep medication and racial groups. Spearman’s correlation (rs) was used to assess the relationship of substance use (alcohol and caffeine), age, elite experience and match performance with selected sleep indices. Statistical significance for all measures was accepted at p < 0.05. Hedge’s (g) were used as the measure of effect size. Results: Light-emitting technology use, effects of travel, late evening alcohol consumption and muscle soreness were the main factors that impacted sleep. Post-match total sleep time (06:31 ± 01:09) was significantly (p < 0.05) shorter compared to post-travel (07:53 ± 01:07; g = 1.19 [0.81;1.57]) and pre-match (08:43 ± 01:03; g = 1.97 [1.55;2.39]) total sleep time. Post-travel sleep onset latency and sleep efficiency were significantly (p < 0.05) shorter (g = 0.74 [0.29;1.29]) and higher (g = 1.35 [0.76;1.94]) at home than away. Although not significant (p > 0.05), allrounders took longer to fall asleep (g = 0.90 [0.23;1.57]), obtained less total sleep (0.76 [0.29;1.42]) and had lower morning freshness scores (g = 1.10 [0.42;1.78]) the night before a match compared to batsmen. Wake after sleep onset and get up time were moderately longer (g = 0.61 [0.22;1.26]) and later (g = 0.62 [0.27;1.17]) before. Twenty20 matches compared to One-Day International matches respectively. Further, sleep duration significantly declined from pre-match to post-match during the multi-day Test format (p = 0.04, g= 0.75 [0.40;1.12]). Late alcohol consumption was significantly (p < 0.05) correlated with a decrease in total sleep time, regardless of match venue (home: rs (49) = -0.69; away: rs (27) = -0.57). During the away condition, an increase in age was significantly associated with longer wake after sleep onset durations (rs (13) = 0.52, p = 0.0003), while greater elite experience was significantly associated with longer total sleep time (rs (72) = 0.36, p = 0.02). The non-sleep medication group took significantly longer to fall asleep compared to the sleep medication group during the first week of the away condition (p = 0.02, g = 0.75 [0.25;1.26]) particularly on nights following transmeridian travel. Although not significant ( p > 0.05), Asian/Indian players had moderately longer sleep onset latencies (g = 1.07 [0.66;1.47]), wake after sleep onset durations (g = 0.86 [0.42;1.29]), and lower subjective sleep quality (g = 0.86 [0.46;1.26]) and morning freshness scores (g = 0.89 [0.47;1.27]) compared to Whites. Similarly, Black Africans had moderately lower subjective sleep quality scores compared to Whites (g = 0.71 [0.43;0.97]). Longer sleep onset latencies and shorter total sleep times were significantly (p < 0.05) associated with poorer One-Day International (rs (28) = -0.57) and Test (rs (12) = 0.59) batting performances respectively. Higher subjective sleep quality scores were significantly associated with better Twenty20 bowling economies (rs (8) = -0.52). Discussion: There was no evidence of poor pre-match sleep behaviour, irrespective of venue; however, the most apparent disruption to sleep occurred post-match (similar to that found in other team-sports). Most disparities in sleep between match venues existed post-travel, with better sleep behaviour observed during the home condition. The differences in sleep patterns found in all three match formats were expected given the variations in format scheduling and duration. Although sleep medication was shown to promote better sleep, its long-term effectiveness was limited. The results promote the implementation of practical strategies aimed to reduce bedtime light-emitting technology use, late evening alcohol consumption and muscle pain. Inter-individual sleep behaviour was found between player roles, age, experience level and race. Moderate associations existed between sleep and markers of batting performance, specifically for the longer, strategic formats of the game. Conclusion: The current study provided new insight of the sleep behaviour in elite South African cricket players during competition. Individualized sleep monitoring practices are encouraged, with specific supervision over older, less experienced players as well as the racial minorities and allrounders of the team. The poor post-match sleep behaviour, together with the sleep and performance correlations, provide ideal opportunities for future interventions to focus on match recovery and the use sleep monitoring as a competitive advantage.

- Description: Musculoskeletal disorders (MSDs) are very common among industrial labourers, leading to sick leave absence, disability and low productivity at work. MSDs have multifactorial causes, thereby making it difficult to pinpoint the extent of the contributions of the various risks regarding the mechanism of its pathophysiology. Previously, attempts were made to study regional pain symptoms in varying occupational groups, disregarding the particularities of the different workplace situations. It is important to highlight, that quantifying physical exposure factors that contribute to MSDs occurrence using quantitative approach alone could be inadequate. It is therefore imperative to incorporate qualitative approach to unmask accompanying psychosocial stressors, which are mostly explanatory variables. The study investigated the following: 12-month retrospective prevalence of MSDs, prospective incidence, risk factors and pattern of MSDs in a 6-month follow-up survey of selected quarry mineworkers, who reported MSDs complaints at the mine clinic. The study also determined the level of psychosocial stress associated with working in the mine by understanding stakeholders’ perception of work challenges, and how it influenced recurrence of MSDs. The study utilised a mixed method design with two phases: phase 1 was an initial baseline retrospective prevalence and a prospective cohort-control incidence survey. The Cultural and Psychosocial Influence on Disability (CUPID) questionnaire was translated into an Igbo-language version, cross-culturally adapted and utilized. The instrument was administered via oral interview and sought information on the following: demographic characteristics; pain in different anatomical sites and associated disability for tasks of daily living; as well as information on any change of job and reason during the study period, recent pain in different anatomical sites and associated disability for tasks of daily living; distress from common somatic symptoms; mental health status, sickness absence in the past 5-months for musculoskeletal pain. Phase 2 was a qualitative focus group discussion that engaged mine stakeholders on their perceptions of work challenges. Descriptive statistics of mean and standard deviation was utilised to analyse sociodemographic characteristics of participants. MSD incidence rates were estimated and the patterns of MSD incidence at different anatomical sites were also determined. The Cox proportional regression analyses were utilised to compare hazard ratios of MSD risks between the exposed cohorts and unexposed cohorts in the phase one incidence survey, alpha level was set at 0.05. Transcription, thematic and content analyses with the aid of Nvivo version 11 for windows was used to interpret stakeholders’ perception of work challenges. The incidence of MSD was 6.2 per 1000, with shoulder pain being the most reported complaint (40% prevalence). This was followed by back pain with the distribution of back pain presented as follows: upper back (20.37%), lower back (45.50%) and both (34.13%). All participants reported previous medical history of back, shoulder and neck pain. The relative risk ratio of mineworkers to MSDs was 4.57, which implies that the mineworkers were approximately 5 times more at risk of developing MSDs compared to the recruited unexposed cohorts. Meanwhile, wrist pain (43.58%) was the highest MSD complaints which lead miners to be absent from work, followed by knee and back with 35.42% and 21% respectively. Physical job risks, such as III working underground and squatting or kneeling at work were all significantly associated with MSDs recurrence in all six body parts (HR: 1.12-1.92, 95%CI). Similarly, time pressure to complete task, low decision latitude, poor job satisfaction, somatisation and mental disorder, job insecurity, poor incentive system (HR: 1.01-1.75, 95%CI) were perceived by stakeholders’ and participants alike in the study to be significantly associated with MSDs incidence. Quarrymen are exposed to high MSD risks associated with elevated psychosocial stress from their difficult work situations. The South Eastern Nigerian mine stakeholders believed that the high incidence of MSDs may have a substantial impact on health and productivity in the near future. Necessitating a clear need for the promotion of stakeholder’s engagement concerning safe work policy implementation initiatives in the sector.

Quantification of pre-competitive sleep/wake behaviour in a sample of South African cyclists

- Description: The quantification of athlete pre-competitive sleep behaviour is of interest owing to the possibility that sleep loss may have a negative effect on health and performance. The purpose of this study was to monitor and quantify the sleep/wake patterns of South African cyclists prior to competitive races. A total of 336 cyclists, male and female and of differing competition levels, cycling in either the 2015 Tsogo Sun Amashova or the 2016 Telkom 94.7 Cycle Challenge completed an altered version of the Competitive Sports and Sleep Questionnaire. The questionnaire asked cyclists to report on precompetitive sleep over the past year. A subset of 92 cyclists also recorded a Core Consensus Sleep Diary for the three nights leading up to the races. The questionnaire showed that 67% of the cyclists reported worsened sleep at least once prior to competition within the past 12 months. The sleep diary found that the cyclists’ average sleep duration the night before the races was 6h19min (±1h38min), which was significantly less than two and three nights prior to the races. Sleep quality was also shown to deteriorate significantly the night before the races. The contributing factors leading to worsened pre-competitive sleep were the time the cyclists had to wake-up as well as perceived increases in sleep latency and awakenings after sleep onset. Anxiety was found to be the major cause of sleep disturbances. While females were found to be significantly more likely to report having experienced poorer sleep before competition in the past year, the sleep diary showed no difference in sleep the night before the races between the sexes. Females were significantly more likely to report instances of unpleasant dreams and waking up during the night. Again, the sleep diary data did not corroborate these findings. Females were also found to report significantly more accounts of nervousness or thoughts about competition as being the cause of sleep problems. There was no difference in sleep loss the night before competition when comparing competition-level groups. The only significant difference was that recreational cyclists were more likely to report sleeping in foreign environments as a cause of sleep disturbances. Despite a large percentage of cyclists experiencing pre-competitive sleep loss, over half (55%) perceived sleep loss to have no impact on their performance. Analysis of pre-sleep behaviour also revealed that the cyclists engaged in several practices that may have a negative effect on subsequent sleep. The vast majority of the cyclists (61%) indicated having no specific strategy to help them sleep the night before competition. Fifteen percent of cyclists reporting using media devices to help them fall asleep, a practice that has been shown to disrupt sleep. In conclusion, most cyclists, regardless of sex and level of competition experience precompetitive sleep loss attributed largely to anxiety but with the perception that this loss in sleep does not negatively impact their performance.
