Policy responses to the sexual and reproductive health of queer youth in the global south: a systematic review

- Description: Concerns surrounding youth sexual and reproduction health (SRH) are deeply embedded within systems of heteronormativity and ciscentrism. Resultantly, youth SRH is filtered through a lens of heterosexual and cisgender experience, rendering invisible the SRH needs of queer youth. Importantly, a failure to recognise queer experiences of SRH has implications for normative subject positions, which enjoy stronger institutional support and constitute legitimate ways of being. As such, the failure to recognise queer youth as health care subjects within policy has far reaching consequences for their sexual and reproductive health. Within this research, a sexual and reproductive justice (SRJ) framework is adopted as a backdrop for exploring policy documents related to youth SRH within selected global South countries in Africa, Asia, and the Caribbean. The final data set comprises of 1035 policy excerpts extracted from 152 policies across these three regions. Research takes the form of a systematic review utilising a deductive framing and positioning thematic analysis. Analysis identifies framings of youth SRH and explores the subject positions assigned to queer youth in relation to these identified framings, with the understanding that the manner in which youth SRH concerns are framed and queer youth are positioned within policies provides an important foundation for the implementation of SRH-related policy. Findings demonstrate that policy responses to youth SRH are most often framed in terms of a public health approach. As a result, dominant understandings of youth SRH serve to reduce youth sexuality to notions of infections and impact, which may speak to an overreliance on biomedical and population-level health models. Themes emerging within human rights framings demonstrate a presumption that rights are equally afforded to, and freely exercised by, all individuals once legally secured, failing to engage with the creation of enabling conditions to realise these rights. Although context and culture framings were by no means exhaustive examples of SRJ, they provide an interesting insight into how such SRJ concerns might be integrated into policy. Importantly, policy responses demonstrate a general pattern hypervisibility of men who have sex with men (MSM) standing in marked contrast to the invisibility of queer youth and other adult queer populations. Within policy extracts, both youth and „MSM‟ are positioned as particularly prone to poor SRH outcomes. By virtue of their inclusion within both populations, queer youth may be considered as especially at risk for, or vulnerable to, such outcomes. Relatedly, these populations (and by extension queer youth) are positioned as in need of correction, containment, and/or protection by those occupying „gatekeeping‟ positions (e.g. health care providers). The positioning of „MSM‟ solely within the context of HIV/AIDS serves to link same-sex sexualities (and at times gender non-conformity) with harmful consequences, suggesting that the positioning of queer youth could similarly serve to conflate their SRH needs with concerns around HIV/AIDS. Many of the subject positions deployed in policies serve to deny the potential for youth and „MSM‟ agency, strength, and resilience. Thus, queer youth subjects are unlikely to be positioned as empowered, autonomous, and agentic. Across both framing and positioning themes, a number of key shortcomings were observed. For the most part, policy responses fail to acknowledge the influence of social, economic, political, and cultural forces that may serve to hinder SRH outcomes according to particular contexts and the intersection of multiple and varied social identities. By obscuring these broader contextual factors and power relations, policy responses may serve to hold individual youth responsible for poor SRH outcomes. In failing to engage with the potential for diversity within youth populations, these populations are largely homogenised. Finally, the need for the creation of an enabling environment in order to secure sexual and reproductive health is largely unacknowledged within policy responses.

Using a social constructionist framework, I explore other ways of talking about social support in an attempt to expand the discourse around social support. I explore how pregnant women talk about social support during pregnancy from their intimate partners in the small rural municipality of Elundini, Eastern Cape, South Africa. Intimate partner support was limited to heterosexual partners regardless of their marital status. The sampling procedure followed a non-probability sampling method. Participants of the study were between 24 and 32 years old. Their gestational age ranged between five and eight months. Fourteen in-depth interviews using photo-elicitation were conducted with seven participants and were analysed using a social constructionist informed thematic analysis. The major theme that emerged from the analysis was partner involvement and absence during pregnancy. The analysis of results suggests that expectant father presence translates to social support. Participants constructed his presence as reassurance in the context of possible abandonment. Absence was constructed in different ways: participants constructed absence as unjust and unfair, absence and marriage, temporary absence in the form of cultural phenomenon of ukwaliswa/ukubukubazana, absence as normal but burdening to the pregnant women’s social network. Participants reported that social support from the expectant father affected pregnancy wantedness.

- Description: Current research in the sub-Saharan and other resource poor contexts indicates the largely negative social constructions of menstruation and menstruating women. Young men have been shown to reproduce these negative constructions and reinforce the stigmatized status of menstruation in these contexts. To my knowledge no studies have examined the ways in which young men talk about menstruation and menstruating women in South Africa. In this research, I aimed to explore the ways in which young men (in a resource poor area in the Eastern Cape) talk about menstruation in with their male peers in a focus group context and how this talk serves to enable specific subject positions (both masculine and feminine) that may reproduce, comply with and resist constructions of hegemonic masculinity (as outlined in previous South African research). By drawing on Raewyn Connell’s influential framework of masculinities and augmenting this with Margaret Wetherell and Nigel Edley’s contributions, this research adds to the growing body of research on masculinities in the South African context. I utilized a discursive framework in which to understand the interpretative repertoires drawn on in everyday talk about menstruation and the specific subject positions made available by these. Purposive sampling was used to recruit a total of 37 participants from two former Department of Education and Training schools in the Eastern Cape. Participants were young ‘black’ men with a mean age of 18.3 In analyzing and interpreting the data two overarching patterns emerged. In the first, the participants discursively distanced themselves from menstruation (and femininity in general) in order to avoid possible marginalisation and subordination in relation to local hegemonic masculine ideals. In doing this, the participants drew on a number of interpretative repertoires including: a dualistic repertoire, a bad (versus ideal) femininity repertoire and an abject femininity repertoire, which assisted in creating numerous subject positions. These subject positions allowed the young men to align themselves closer to hegemonic masculine ideals, and create distance by positioning menstruating women as the ‘other’. In the second overarching pattern, menstruation was constructed as a threat to masculine identity; within this construction, the young men discursively negotiated the ideological dilemmas surrounding this ‘highly feminine’ topic in ways that bolstered their positions within the gender hierarchy. Overall, hegemonic masculinities in this context were discursively reproduced and complied with in the participants’ accounts.

Sistering and sexual socialisation: a psychosocial study of Xhosa women’s ‘sex and reproduction talk’ with their sisters

Moreover, I examine why these women emotionally invest in certain discourses over others in their sisterly sex talk. The psychosocial framework combines discursive psychology with a psychoanalytic approach to explore both ‘inner’ and ‘outer’ processes involved in sexual socialisation within sistering. Psychoanalytic concepts were used to ‘thicken’ the discursive reading of the text. Five isiXhosa-speaking, middle aged and working class women were the participants of the study. The Free Associated Narrative Interview was used to collect the data thus participants were encouraged to convey their experiences of sex talk and reproduction within their sisterly relationships with minimal interruptions from the interviewer. Analysis suggests that the women drew on two broad interpretative repertoires in order to construct their sisters as sexual subjects: a repertoire of secrecy, and a repertoire of responsibilisation and risk. Furthermore, sisterly sex talk was constructed as a complex interchange of shifting positions of openness and power due to two coinciding identities - that of mother and of peer taken up by the participants. A psychoanalytic reading of the data suggests an emotional investment in exemplifying responsibility. The participants experienced anxiety in negotiating a sexualised femininity and projected their sexual desire onto their sister to defend against anxiety in their talk; this enabled them to feel as though they were ‘doing’ sex better than their sisters.

A narrative-discursive analysis of abortion decision-making in Zimbabwe

Research on abortion decision-making has rarely examined how women who undergo a termination of pregnancy (TOP) construct micro-narratives of the decision to terminate the pregnancy and also how these women are positioned by the service providers who interact with them. Using a Foucauldian postcolonial feminist approach and narrative-discursive analysis, this study explores abortion decision-making narratives in a Zimbabwean context where abortion laws are restrictive. In this study I elicited the narratives of women who had undergone an abortion about how they came to make the decision and proceeded to terminate the pregnancy. I highlight the discourses employed in constructing these narratives and how women position themselves in these narratives and discourses. These are then compared to the subject positions enabled in health service providers’ narratives on the same topic. These narratives are then linked to the social discourses and power relations that work to enable or constrain reproductive justice. The data were collected from three sites in Harare, Zimbabwe. The three sites were Harare Hospital, Epworth and Mufakose. An adapted version of Wengraf’s (2001) narrative interview was used to elicit narratives from 18 women who had terminated pregnancies (six at each site). Semi-structured interviews were conducted with six service providers (two nurses at Harare Hospital, two village health workers in Epworth and two nurses in Mufakose). All the service providers interviewed have experience working with women who have terminated pregnancies. In narrating their stories about their abortions, the women employed discursive resources around shame, stigma, religion, health and culture. These discursive resources were drawn upon in the construction of the women’s micro-narratives. The women spoke in a socially sanctioned manner where stories were enabled and constrained by particular religious, cultural and gendered discursive resources. In these stories, cultural constructions, gendered understandings of motherhood and femininity constrained reproductive justice for women who have terminated pregnancies. Comparisons of the way women positioned themselves and how they were positioned by health service providers point to the existence of social discourses and power relations that work to constrain reproductive justice. While the women saw themselves as having ‘unsupportable pregnancies’, the service providers positioned them as being evil, selfish and irresponsible. The negative positions deployed by the service providers point to the vilification and blaming of women who have undergone a termination of pregnancy. In these positions, the woman is at fault and there is silence on the role of men in abortion decision-making. In the women’s narratives and the health service providers positioning of the women a ‘reproductive rights’ discourse was absent. This was significant as much of the activism around abortion has centred on the woman’s rights to her body. Where rights were mentioned, it was in reference to foetal rights (using cultural, moralistic religious understandings of abortion as killing). The missing ‘reproductive rights’ discourse points to a need to move from a reproductive rights framework to a reproductive justice framework that can be applied through local understandings of hunhu/ubuntu. By doing this, abortion is not seen as a ‘choice’ that a woman makes but rather as involving broader social and environmental circumstances that make a pregnancy ‘unsupportable’.

In these stories, cultural constructions, gendered understandings of motherhood and femininity constrained reproductive justice for women who have terminated pregnancies. Comparisons of the way women positioned themselves and how they were positioned by health service providers point to the existence of social discourses and power relations that work to constrain reproductive justice. While the women saw themselves as having ‘unsupportable pregnancies’, the service providers positioned them as being evil, selfish and irresponsible. The negative positions deployed by the service providers point to the vilification and blaming of women who have undergone a termination of pregnancy. In these positions, the woman is at fault and there is silence on the role of men in abortion decision-making. In the women’s narratives and the health service providers positioning of the women a ‘reproductive rights’ discourse was absent. This was significant as much of the activism around abortion has centred on the woman’s rights to her body. Where rights were mentioned, it was in reference to foetal rights (using cultural, moralistic religious understandings of abortion as killing). The missing ‘reproductive rights’ discourse points to a need to move from a reproductive rights framework to a reproductive justice framework that can be applied through local understandings of hunhu/ubuntu. By doing this, abortion is not seen as a ‘choice’ that a woman makes but rather as involving broader social and environmental circumstances that make a pregnancy ‘unsupportable’.

A partial normalisation of some ‘hidden’ aspects of sex, particularly around issues pertaining to female sexuality, was enabled, and critical consciousness around the gendered inequities in ‘cheating’ was promoted. However, abstinence was relatively silenced, and male same-sex remained heavily stigmatised. No substantial action component beyond the group meetings was generated. Participant feedback indicated that they placed great value on the dialogical processes in the group, and that they enjoyed being able to talk about sexual and other personal aspects of their lives. I theorise that the value of the group was in the recognition that participants received as they were positioned in a variety of subject positions. Whilst dialogue was shown to be extremely valuable, there were suggestions that other, non-dialogical modes of recognition were also needed by participants.

I collected data from two schools in the Eastern Cape that included: (i) sections on sexuality from two Life Orientation manuals used by educators in classrooms: ‘Oxford Successful Life Orientation’ (2011), and ‘Shuters Top Class Life Orientation’ (2011); (ii) videos and lyrics of three songs voted most popular by learners which were ‘Climax’ by Usher, ‘Beez in the Trap’ by Nicki Minaj, and ‘Where Have You Been’ by Rihanna; (iii) observations of seven sexuality education classes; and, (iv) in-depth semistructured interviews conducted with eight learners and two educators. I draw on an integrated theoretical and methodological approach – Foucauldian, feminist poststructural and psychosocial psychoanalytic perspectives – to conceptualise and analyse gendered sexualities in terms of: (i) the dominant gendered discourses found in sexuality education manuals, and music videos and lyrics; (ii) the reflexive and interactive gendered subject positions taken up and/or resisted by learners and educators during classroom lessons and one-on-one interviews; and, (iii) learners’ and educators’ conscious and unconscious investments in particular gendered subject positions during one-on-one interviews. These three sets of analysis produced four major themes. The first theme centres on responsible sexuality; young women are expected to assume more sexual responsility than young men, thus curbing their sexual agency. The second theme outlines three types of pleasure – sexual, romantic and dating and/or relationship pleasure – that accord young men and women active and passive ways of exercising pleasure. The third theme highlights the heteronormative transitioning adolescent subject that constructs young women as reproductive subjects and young men as sexual subjects. The last theme focuses on gendered power relations and raunch culture, and maintains that young men are powerful and likely to commit acts of sexual violence against young women because they are powerless. The central argument developed when viewing all the themes is that dominant gendered discourse, gendered subject positions, and conscious and unconscious investments in these positions challenge the extent to which the gendered meanings that underpin adolescent learners’ sexuality are stable and fixed. The gendered discourses in the Life Orientation sexuality education programmes showed that gender is expressed rigidly, thus privileging masculine over feminine sexuality. However, the gendered discourses in the popular music contested rigid gender binaries and produced fluid and equitable masculine and feminine sexualities. The classroom practices depicted multiple and more equatable gendered sexualities, highlighting just how contested gender is. Finally, educator and learners’ personal biographies illustrated how conflicting masculine and feminine sexualities present a signficant source of emotional conflict for them. It may benefit policymakers and stakeholders to consider informal mediums of sexual socialisation for learners, such as music, when drafting the Life Orientation sexuality education curriculum, whilst also taking into account learners and educators personal lives.

Counsellors’ talk about their understanding of, and practices in response to, intimate partner violence during pregnancy: a narrative-discursive analytic study.

- Description: South Africa is a very violent society, where violence is often used as a social resource to maintain control and establish authority. Global and local research suggests that there is a high prevalence of intimate partner violence (IPV), a facet of this violence, although little research has been conducted into the effects of IPV during pregnancy in the South African non-governmental organisation (NGO) context. NGOs globally and in South Africa have attempted to address IPV and IPV during pregnancy by providing services that aim to assist (largely female) clients emotionally and logistically. In light of this phenomenon, this qualitative study presents data generated through the use of a lightly-structured narrative interview schedule. The interviews were conducted over three sessions with eight counsellors, all based at two NGOs in South Africa and experienced in counselling women who have suffered IPV and IPV during pregnancy. This study used Taylor and Littleton’s (2006) narrative-discursive analytical lens, infused with theoretical insights from Foucault about power, discourse and narrative in order to identify the discursive resources that shape the narratives that the counsellors shared in the interviews and how these translate into subject positions and (gendered) power relations of the men and women about whom they speak. Six discursive resources emerged from the narratives, namely a discourse of ‘traditional “African” culture’, ‘patriarchal masculinity’, ‘nurturing femininity’, ‘female victimhood’, ‘female survivorhood’ and ‘human rights’. These informed the three main narratives that emerged: narratives about IPV in general, IPV during pregnancy, and the counsellors’ narratives about their intervention strategies. Within these narratives (and the micro-narratives which comprised them), men were largely positioned as subscribing to violent patriarchal behaviour whilst women were mostly positioned as nurturing and victims of this violence. The counsellors also constructed women as largely ignorant of their options about IPV and IPV during pregnancy; they constructed these phenomena as problems that require intervention and identified a number of factors that indicate what successful IPV interventions should entail. In reflecting upon this analysis, this study also aimed to address the questions of what is achieved or gained by using these narratives and discursive resources, what the significance or consequences are of constructing and using these particular narratives and discourses and whether different narratives or discourses would have been possible. Recommendations for further research includes incorporating more sites as well as interviewing perpetrators and IPV survivors themselves, perhaps in their home language where relevant rather than English, to gain a broader and more faceted understanding of the dynamics surrounding IPV during pregnancy. A recommendation for practice in intervention against IPV during pregnancy is to introduce more holistic/systemic intervention strategies and working with communities to address this issue.

Purposive sampling was used to recruit a total of 25 participants from three different abortion facilities in the Eastern Cape. Participants were ‘Black’ women, mostly unemployed and unmarried with ages ranging between 19 and 35 years old. In analysing and interpreting participants’ narratives, the picture that emerged was an over-arching narrative in which women described the abortion decision as something that they were ‘forced’ into by their circumstances. To construct this narrative, women justified the decision to have an abortion by drawing on discourses that normalise certain practices located within the husband-wife and parent-child axes and make the pregnancy a problematic, unsupported and unsupportable one. While South African studies of young people‘s talk about sexualities have been conducted, there is a paucity of literature in this area from a discursive perspective. This study is situated within a feminist post-structuralist paradigm, utilising a performative-performance analytical approach which synthesises Butlerian theory with a narrative-discursive methodology. This approach enables an analysis of both the macro-discursive power webs within which sexualities are situated, and the micro-discursive activity through which sexual subject positions are constructed. I used this approach to analyse the talk of groups of students from a Further Education and Training College about the sexualities of High School learners and their own past sexuality education. Findings showed that that the most dominant discursive resources which were utilised to construct sexualities were societal sexual norms discourses. These foundational discourses constructed gendered sexualities of compulsory hyper-heterosex for men, and compulsory compliant girlfriendhood for women. Such gendered sexualities reinforced patriarchal and abusive gendered and sexual practices. Ways in which participants troubled the dominant gendered sexualities through the performance of alternative sexual positions were analysed, as these 'troubling' performances indicate mutable aspects of the normative gendered field. Participants drew on a discourse of disconnect when talking about their school sexuality education, and their parents‘ (lack of) communication with them about sex. This suggests that adultist attempts to construct a 'responsible' sexual subject position for young people are resisted when such a position is constructed in a non-relational manner. Collusion between the constructed gendered sexualities and the discourse of disconnect results in the un-performability of a 'responsible' sexual subject position. These findings were used to provide suggestions for enhancing school based sexuality interventions.

Collusion between the constructed gendered sexualities and the discourse of disconnect results in the un-performability of a 'responsible' sexual subject position. These findings were used to provide suggestions for enhancing school based sexuality interventions.

‘Gender’ and constructions of spousal mourning among the AmaXhosa in the Eastern Cape

The discriminatory nature of the practice, especially pertaining to visibility and some of the detrimental effects on the widows’ health and safety have been documented by some researchers, but the reasons for the continuity of visibility remain largely unexplored. Taking into account the dynamic nature of ‘culture’, this research explored the discourses deployed in men and women’s constructions of ukuzila specifically focusing on spousal mourning and the continuity of widows’ visibility in spite of their resistance to it. The research used postcolonial feminism drawing on postructuralism as its theoretical lens. This theoretical lens provided useful concepts such as hybridity, visibility, surveillance and power with which to examine spousal mourning and conceptualised people’s subject positions as multiple, fluid and contingent. Furthermore, the research employed thematic and discourse analysis at its methodology. Discourse analysis was employed to identify and analyse the discourses utilised in the constructions of spousal mourning. The research was conducted through focus group discussions held with younger and older urban and rural men and women, as well as interviews held with widows and widowers and key cultural informants. Concerning the question of constructions of spousal mourning for men and women, visibility of the mourner emerged as a central and contentious issue. Some participants were of the view that one could show mourning by engaging in culturally appropriate mourning behaviour, whilst others were of the view that showing one’s mourning had to be visible by publicly displaying mourning through a marker. Another group proposed mourning “by heart”, whereby the mourners’ status could either be inferred from their behaviour, whereas others maintained that behaviour was not mandatory. Various justifications for the continued visibility of widows were advanced. These justifications included showing love and respect to the deceased husband; showing respect to the ancestors; and helping to monitor their own behaviour in order to ensure that it is in line with appropriate mourning behaviour. The continued visibility of widows was also used to regulate the widows’ sexuality. Widows were coerced to put on ‘clothes of mourning’ in order to ‘protect’ them from being approached by men for a relationship during the mourning period. The regulation of the movement of widows was also managed through visibility. Widows’ movements were restricted in order to protect the community from pollution or bad luck. For example, they were not allowed to visit places of entertainment or visit other households. Key discourses identified were the familial-‘ukwenda’, respect-‘hlonipha’, and male sexual drive (MSD) discourse. The familial - ‘ukwenda’ discourse is centred on the idea that one is ‘married to the household’, which includes the nuclear family and wider extended family including ancestors. According to the respect-‘hlonipha’ discourse, respect is due to others on the basis of their age, status, and more especially their gender. Showing respect (hlonipha) necessitates the avoidance of all forms of behaviour and utterances that could be deemed disrespectful. The MSD holds a widespread view of sexuality as a biological drive that resides within each male and it was drawn on to make sense of discontinued visibility among widowers, whilst visibility of widows continued. It is argued that it is these discourses, embedded in the ‘culture’ of the AmaXhosa and upheld by the family that sustain the discriminatory nature of the practice, especially concerning the continued visibility of widows in spite of the resistance that has been voiced.

- Description: Despite the increased focus on men in reproductive research, little is known about male involvement in the initial decision/s regarding parenthood (i.e., to become a parent or not) and the subsequent decision-making that may ensue (e.g., choices about timing or spacing of births). In particular, the parenthood decision-making of “White”, heterosexual men from the middle class has been understudied, as indicated in the existing literature. In South Africa, this oversight has been exacerbated by the tendency for researchers to concentrate on “problematic” men, to the exclusion of the “boring, normal case”. I argue that this silence in the literature is a result of the taken for granted nature of parenthood in the “normal” heterosexual life course. In this study, I have turned the spotlight onto the norm of “Whiteness” and heterosexuality by studying those who have previously been overlooked by researchers. I focus on “White” Afrikaans men’s involvement in parenthood decision-making. My aim was to explore how constructions of gender inform male involvement in decision-making, especially within the South African context where social transformation has challenged traditional conceptions of male selfhood giving rise to new and contested masculine identities and new discourses of manhood and fatherhood. In an effort to ensure that women’s voices are not marginalised in the research, as is often the case in studies of men and masculinity, I conducted in-depth, semi-structured interviews about male involvement in decision-making with both “White” Afrikaans women and men. There were 23 participants in total, who all identified as heterosexual and middle-class. The participants were divided into two age cohorts (21 – 30 years and >40 years), which were then differentiated according to gender, reproductive status, and relationship status. Treating the interviews as jointly produced narratives, I analysed them by means of a performativity/performance lens. This dual analytic lens focuses on how particular narrative performances are simultaneously shaped by the interview setting and the broader discursive context. The lens was fashioned by synthesising Butler’s theory of performativity with Taylor’s narrative-discursive method. This synthesis (1) allows for Butler’s notion of “performativity” to be supplemented with that of “performance”; (2) provides a concrete analytical strategy in the form of positioning analysis; and (3) draws attention to both the micro politics of the interview conversation and the operation of power on the macro level, including the possibility of making “gender trouble”. The findings of the study suggest that the participants experienced difficulty narrating about male involvement in parenthood decision-making, owing to the taken for granted nature of parenthood for heterosexual adults. This was evident in participants’ sidelining of issues of “deciding” and “planning” and their alternate construal of childbearing as a non-choice, which, significantly served to bolster hetero-patriarchal norms. A central rhetorical tool for accomplishing these purposes was found in the construction of the “sacralised” child. In discursively manoeuvring around the central problematic, the participants ultimately produced a “silence” in the data that repeats the one in the research literature.

Information about dressing-up, cross-dressing and drag performance was obtained in interviews, from documentary evidence, and from audio-visual recordings of drag shows and gay and lesbian beauty pageant competitions. Semiotics and a Foucauldian approach to analysing discourse were used to interpret the written, spoken, and visual texts. In this study I argue that the state prohibition of homosexuality during apartheid meant that people could not admit to knowing about it, and this ‘not knowing’ provided a cover for homosexual behaviour in public. At this time, the threat of being identified was associated with police raids on private parties. In the 1990s, homosexual visibility was more viable than it had been in the past. However, the strategies that were adopted to negotiate public visibility at this time were tailored to appease normative sentiments rather than challenge them. I argue that, historically, race and gender have played a role in diminishing and exacerbating homosexual visibility and its politics. Addressing the potential for harm that is associated with homosexual visibility in the 21st century post-apartheid context, this study considers the circumstances in which invisibility is desirable.

Public discourses on choice of termination of pregnancy in a rural area of the Eastern Cape Province in South Africa

- Description: A period of ten years has elapsed since the Choice on Termination of Pregnancy Act came into effect. Little has been done in South Africa to investigate public discourses concerning termination of pregnancy since the implementation of the Act. The social context and the quality of available support systems determine the outcome of the women’s feelings after the abortion. Knowledge about the social context is important, as it will help to understand the complexities and nuances of abortion. The aim of the research is to explore public discourses on Choice on Termination of Pregnancy (CTOP), and the potential implications of these discourses on the use of the CTOP service. The sample consisted of 23 black isiXhosa-speaking participants from the rural area of the Eastern Cape Province in South Africa. Four focus groups coming from different age groups (between the ages 18 and 52) with both men and women participated in the study. Fictitious vignettes that tap into two different scenarios regarding abortion based on women’s stories were used. Discourses that emerged from people’s text are explained, described and interpreted through a discourse analysis. Since the study was interested in public discourses it led to the discovery of 17 interpretative repertoires as follows: social stigma, abortion equated to murder, degradation of society, pregnancy as an irresponsible act, conditional acceptance, TOP in the context of marriage, future potentiality invested in the foetus, dehumanizing foetus into a clot, shared decision making responsibility, gender dynamics interpretative repertoire, negative post abortion consequences, the scolding versus the supportive nurse interpretative repertoire, alternatives interpretative repertoire, rights versus no responsibility interpretative repertoire, more knowledge needed, male and female or generational differentiation repertoire, and the positive effects repertoire. Abortion is opposed on religious and cultural grounds. TOP has been legalized in South Africa but with this a debate and conflicting views have arisen. These variations in people’s discourses may limit access to TOP for women who need the service.

In the ten years since the 'new' democratic South Africa emerged, transformation has become a dominant discourse that has driven much action and practice in a variety of public areas. One of the areas of focus for transformation has been Higher Education whereby the Department of Education aimed to do away with disparity caused by Apartheid segregation by reducing the number of Higher Education institutions from 36 to 21. This research draws on Foucauldian theory and post-colonial theories (in particular Edward Said and Frantz Fanon), and the concept of racialisation in an analysis of the incorporation of Rhodes University's East London campus into the University of Fort Hare. Ian Parker's discourse analytic approach which suggests that discourses support institutions, reproduce power relations and have ideological effects, was utilised to analyse the talk of students and staff at the three sites affected by the incorporation (viz. Rhodes, Grahamstown, Rhodes, East London and Fort Hare) as well as newspaper articles and public statements made by the two institutions. What emerged was that in post-Apartheid South Africa, institutional and geographic space is still racialised with virtually no reference to the historical and contextual foundations from which this emerged being made. In positioning space and institutions in this racialised manner a discourse of 'white' excellence and 'black' failure emerges with the notion of competence gaining legitimacy through an appeal to academic standards. In addition to this, transformation emerges as a signifier of shifting boundaries in a post-Apartheid society where racialised institutional, spatial and social boundaries evidently still exist discursively.

Rhodes, Grahamstown, Rhodes, East London and Fort Hare) as well as newspaper articles and public statements made by the two institutions. What emerged was that in post-Apartheid South Africa, institutional and geographic space is still racialised with virtually no reference to the historical and contextual foundations from which this emerged being made. In positioning space and institutions in this racialised manner a discourse of 'white' excellence and 'black' failure emerges with the notion of competence gaining legitimacy through an appeal to academic standards. In addition to this, transformation emerges as a signifier of shifting boundaries in a post-Apartheid society where racialised institutional, spatial and social boundaries evidently still exist discursively.
