The mechanisms through which this comes about are unclear. Murray & Cooper (1997) suggest an impaired pattern of mother-infant communication is responsible. Within this, Murray proposes that maternal speech may be a key factor. This study constitutes a preliminary exploration into the mechanisms through which maternal depression effects mother-infant interaction in South Africa. 147 predominantly Xhosa-speaking mother-infant dyads that took part in a broader epidemiological study on post-partum depression in Khayelitsha (Cooper, Tomlinson, Swartz, Woolgar, Murray & Molteno, 1999) made up the subjects. Maternal depression was assessed according to the Structured Clinical Interview for DSMIV (SCID). Maternal speech recorded from standard, five-minute, face-to -face mother-infant interactions was translated and analysed according to a coding system developed by Murray (Murray, Kempton, Woolgar & Hooper, 1993). The speech of depressed mothers to their two month old infants was compared to the speech of non-depressed mothers on dimensions of focus, affect and agency, and the role of infant gender was assessed. Results revealed no significant group differences for depression. Maternal speech to male infants was found to hold significantly less ascription of agency than to female infants. Findings suggest that maternal speech may be too narrow a marker of maternal depression in this context and that broader indices are needed. It is recommended that future research control for measures of social adversity, factor in cultural and language particularities, and consider contextual aspects of mother-infant interaction / attachment processes, in investigating the mechanisms through which post-partum depression leads to negative infant outcome in the developing world.

- Description: This study explores the history of sex communication and education over a period of fifty years (1950 to the present), in a deep rural area of the Eastern Cape. It describes patterns of sex communication between peers, between siblings, between children and parents and between young people and other non-familial agencies within communities. Communication trends are traced from the period before contraception was introduced, through to the introduction of female birth control methods into the HIV/AIDS era where the focus has been on attempting to introduce condoms. Twelve semi-structured interviews and two four-person focus groups were conducted to gather information on how the participants acquired information about sexuality and their responses to the same. The findings of the present study suggest that the widespread use of injectable contraceptives has had a marked effect on the sexual culture of the community under investigation. It has led to the collapse of the regulatory practices which were previously in place. This in turn has significantly affected the sexual communication and negotiation context. Furthermore, it has had a determining influence on male involvement in sexual reproductive health matters and has created a poor context for the adoption of condoms as a prophylactic. There were no major changes in the sexual communication context within families and within communities in that education has always been limited to instructions to avoid pregnancy. Of note was a culture of collusion between adults and children surrounding sexuality, which absolved the parties involved in addressing sexuality. These factors are understood have mediated response to HIV/AIDS prevention efforts, and need to be taken into consideration in the development of sex communication and education programmes.

Exploring differences between organisational cultures in a company undergoing change

- Description: Change is a common phenomena in business organisations today. The turbulent environment is exerting a significant influence on organisations to make continuous changes in their internal environment in an attempt to improve performance and achieve competitiveness in the marketplace. The human factors which confront organisations undergoing change have not been thoroughly investigated in Sub-Saharan Africa. The present study aimed at investigating human issues affecting the performance ofan organisation located in an Afiican country. Data was collected using semi-structured interviews and focus groups. Twenty interviews were conducted with employees randomly selected from all the levels of the company, namely attendant, operator, supervisory, middle and top management. Five focus groups were carried out with employees from different employee categories. Both sets of data were analysed using grounded theory. The findings showed that there were several problems in the company including insecurity, lack of opportunities for employee development, autocratic management style, lack of participation in decision making, substandard performance, ineffective human resources systems and lack of adaptation to technology. It was suggested that the problems resulted from the clash between organisational cultures in the company. At one level, there was a clash between the local workers' socialist organisational culture and the expatriates' capitalistic organisational culture. At a more deeper level, the clash appeared to be between the local employees' collectivistic sociocultural values and the expatriates' individualistic value system. These findings have significant implications for managing change in organisations with diverse cultures.

- Description: The dominant emphasis in Western models of bereavement is on the breaking of bonds with the deceased in order for healing to occur. Failure to let go often leads to a diagnosis of 'pathological grief'. This paper challenges the assumption that death invariably means that the bonds with the deceased have to be severed. Situating Western models of bereavement in a modernist context not only challenges the 'truth' claims of these models, but also facilitates a deconstruction of the elements that contribute to the emphasis on letting go. In contrast to these theories, two Xhosa rituals (umkhapho and umbuyiso) that seek to sustain the bond with the deceased person will be examined. Such rituals demonstrate that it is possible to both maintain the bond and for the bereaved person to move on with their lives. Despite different contexts, it will be argued that these Xhosa bereavement rituals have a contribution to make to Western models of bereavement and some implications for therapy will be explored.

The research showed that nurses experience two forms of change. It was suggested that nurses respond to external changes in a manner designed to maximise their control and sense of meaning in an unpredictable environment. Through interviews and focus group discussions, the enculturation of youth into sexual activity in the community of interest is explored. The study looks into sexual experiences from childhood, through early adolescence to adults of up to sixty five years old, allowing an analysis of the changing forms of sexual experimentation and sexual debut in this context. Contextual factors mediating these changes are explored, with special emphasis on the changing regulatory practices around early sexual experiences and the effect thereof on behaviors connected to HIV infection risk. Practices that were previously important mediators of sexual behavior were: anxiety about the social consequences of pregnancy, which was previously a significant disincentive to sexual intercourse; men's previous acceptance of the need to practice non-penetrative forms of sex and girls postponing sexual debut for as long as possible. All these were culturally endorsed, but are now noted to have changed due to changes in the regulatory practices surrounding youth sexuality. The context of early sexual experiences and the surrounding cultural practices have also changed significantly, and this needs to be taken into account in understanding receptivity to condom use messages. Implications for HIV/AIDS prevention are discussed. Among other interventions, the study describes a participatory, community-based, multi-sectoral approach that takes social conditions into account as a way of empowering the community to strengthen its response to the pandemic. This incorporates different sectors of the community, including youth, parents, religious and traditional leaders, and various other structures, services and institutions that make up the community.

- Description: This study is a case-study of a patient who was diagnosed as having co-morbid Anorexia Nervosa and Factitious Disorder. It would appear that central to an understanding of both of these disorders is the patient's disturbed relationship to her own body. The existing literature on co-morbid Factitious Disorder and Anorexia Nervosa is rare, with only three cases published. A careful reading of these cases suggests that in all of these cases, Factitious Disorder may have been the primary diagnosis. In this case there was as overidentification with the patient role, and the patient's anorexic symptoms appeared to serve the function of meeting the patient's acute dependency needs. Thus, the patient's Anorexia Nervosa masked the Factitious Disorder and appeared to be secondary to the Factitious Disorder. It is argued that these disorders share several common dynamics, namely the inability to separate from the mother, high parental expectations as well as the use of the body as a transitional or pre-cursor object. In addition there are some common dynamics in the psychotherapy of these patients. All of these factors suggest that in both these disorders the developmental arrest may be located at a pre-verbal level.

- Description: The pandemic of HIV/AIDS has challenged several aspects of contemporary social life. HIV/AIDS counselling has developed as a social response to provide support for those infected with the disease. Due to the nature and complexity of the disease, HIV/AIDS counsellors encounter a diversity and intensity of emotions when counselling. A support system that includes effective management and supervision may assist in resolving emotions and reactions that HIV/AIDS counsellors may experience as a result of working with HIV/AIDS clients. This study examines the difficulties that both managers and the HIV/AIDS counsellors at a rural district hospital in the Eastern Cape and its surrounding clinics are faced with, in providing the HIV/AIDS counsellors with the support they need. Engestrom's (1987) model of activity theory was used as a conceptual framework guiding both the analysis and interpretation of the data. This model facilitates the identification of tensions and contradictions thereby opening a space for change and transformation within an activity system. Multiple sources of data collection were used that included focus groups with managers and HIV/AIDS counsellors, interviews with senior hospital staff and an official from the Department of Health, Eastern Cape, and a tour of the rural district hospital. The findings suggest that HIV/AIDS counsellors do not feel supported by their managers. Managers in turn are of the opinion that they do not offer the support the HIV/AIDS counsellors' need. The lack of support is attributed to geographic distances between hospitals and the clinics they serve, lack of available transport and the multiplicity of roles of both managers and the HIV/AIDS counsellors. The pandemic of HIV/AIDS has challenged several aspects of contemporary social life. HIV/AIDS counselling has developed as a social response to provide support for those infected with the disease. Due to the nature and complexity of the disease, HIV/AIDS counsellors encounter a diversity and intensity of emotions when counselling. A support system that includes effective management and supervision may assist in resolving emotions and reactions that HIV/AIDS counsellors may experience as a result of working with HIV/AIDS clients. This study examines the difficulties that both managers and the HIV/AIDS counsellors at a rural district hospital in the Eastern Cape and its surrounding clinics are faced with, in providing the HIV/AIDS counsellors with the support they need. Engestrom's (1987) model of activity theory was used as a conceptual framework guiding both the analysis and interpretation of the data. This model facilitates the identification of tensions and contradictions thereby opening a space for change and transformation within an activity system. Multiple sources of data collection were used that included focus groups with managers and HIV/AIDS counsellors, interviews with senior hospital staff and an official from the Department of Health, Eastern Cape, and a tour of the rural district hospital. The findings suggest that HIV/AIDS counsellors do not feel supported by their managers. Managers in turn are of the opinion that they do not offer the support the HIV/AIDS counsellors' need. The lack of support is attributed to geographic distances between hospitals and the clinics they serve, lack of available transport and the multiplicity of roles of both managers and the HIV/AIDS counsellors. Recommendations include the introduction of self-supervision , peer groupsupervision for HIV/AIDS counsellors.

Intrapreneurship is increasingly becoming a term used in the business world to describe organisations that are willing to pursue opportunities, initiate actions, and emphasise new, innovative products or services. Due to the dynamic nature of modern organisations, it is imperative that organisations and their managers remain receptive to new ideas, approaches and attitudes. It is therefore the belief that rapid and cost-effective innovation is the primary source of lasting competitive advantage in the twenty-first century, leaving organisations no alternative but to become intrapreneurial or cease to exist. This thesis focuses on this need and examines ways in which intrapreneurship can be measured in organisations in order to provide a benchmark for further organisational development. A questionnaire (known as the Intrapreneurial Intensity Index) was designed and distributed to a sample of 500 employees working in large South African organisations, which classified themselves as ‘forward-thinking’ and aimed for an intrapreneurial ‘type of thinking’. The results obtained from these questionnaires underwent item analysis, after which the questionnaire was redesigned in an electronic format. A pilot case study was then conducted in order to test the reliability of the instrument. Finally the questionnaire was redistributed to a sample of six organisations that are viewed as being ‘intrapreneurial’ and two that are regarded as being ‘non-intrapreneurial’. The data from this sample was used to test the validity of the Intrapreneurial Intensity Index and to demonstrate its application. This study resulted in an instrument that can be used to ascertain the intensity of intrapreneurship present in a large organisation. Specifically, this instrument can provide an overall view of the organisation’s intrapreneurial ability, as well as identify the specific areas in the organisation that require change or modification in order to become more intrapreneurial. This instrument provides a valuable means of identifying areas in need of organisational change, by determining an organisation’s intrapreneurial properties in the organisation’s core areas.

The effects of age and education on selected cognitive tests: the trail making test, the digit symbol sub-test, and the finger tapping test

This study examined the effects of age and education on the Trail Making Test, the Digit Symbol Sub-Test, and the Finger Tapping Test in a non-clinical sample of community dwellers with a relatively low level of education (8 to 12 years) in South Africa. The sample consisted of 161 participants across six age groups: 20-39, 40-59, 69-69, 70-79, 80-89 and 90-95 years. Results were examined for mean age effects and variability trends. Highly significant age effects were present across the age groups for all tests, however, there was no uniform pattern of variability across the tests. The Digit Symbol Substitution Test and the Finger Tapping Tests showed a pattern of increasing variability with increasing age, followed by a decrease in very old age while no trend was evident for the Digit Symbol extensions (the Immediate and Delayed Recall tests). The Trail Making Test, Parts A and B, showed a consistent trend of increasing variability across the age groups. Data from the present study was compared with existing data from two relatively high education samples, with equivalent age groupings, to examine education effects. Results showed an education effect for all tests with the high education groups outperforming the low education groups. Although the effects of education became less potent with advancing age, the mean performance of the oldest (80-89 years) high education age group was superior to that of the equivalent low education age group. Comparison of variability trends across both samples showed that the highest variability (the shuttle bulge) was present at the same point along the age axis, or at a later point, for the low education group, as that for the high education group. This finding is inconsistent with Jordan's (1997) 'shuttle model of variability' which predicts an earlier occurrence of the shuttle bulge (left shuttle shift effect) for a low education sample. This study demonstrated that performance on neuropsychological tests is influenced by age and education and highlighted the dangers inherent in unquestionably applying norms, which have not been corrected for age and education, when assessing the older adult.

The study focuses on two female participants, both with diagnosable psychopathology, who were part of the same MBSR programme at the Cape Town Medi-Clinic. Quantitative self-report measures of depression, anxiety, and medical symptoms were used as a measure of change. In-depth qualitative data which explored psychological, emotional and behavioural changes came from semi-structured interviews taken before, during, and immediately after the MBSR, and at a one-month follow-up. - Description: Diversity training programmes are formal efforts to prepare the workforce to work with individuals from different cultural backgrounds and to improve organisational effectiveness. Although many studies have described diversity training programmes in the workplace, only a few have been evaluated to assess their effectiveness and impact on job outcomes. This thesis describes a study that assessed the impact of a diversity management-training programme on employee attitudes and behaviour towards diversity in a manufacturing company in Namibia. Kirkpatrick’s (1959) four-level model, which examines the trainees’ reactions to the training, the learning acquired, the behaviour change and improvement in organisational results, was used to measure the impact of the training programme. Data was collected through pre-and post-assessment semi-structured individual interviews and a focus group was conducted two months after the training. Data analysis indicates that the first two levels of the evaluation model showed an impact. The participants had positive reactions towards the course and said that they had learnt from the course. The data also showed that the training did not have any impact on the behaviour of the participants and on organisational outcomes. The research also revealed that lack of improved productivity and organisational results might have been influenced by unrealistic expectations, past political conditions, job insecurity and unemployment and the training context. Some recommendations for both the diversity training programme administrators as well as the management of the company are made.

In an attempt to answer this, the researcher asked three questions surrounding the succession process and its relationship with organisational culture. The questions related to the extent to which the family business was prepared for change in management; what changes occurred before, during, and immediately after the succession process, and what results these changes had on the organisational culture. Family businesses account for a large proportion of the national and global market activities, and so it is imperative that attention be paid to any problems they may experience. As it is, family businesses have difficulty in successions, with roughly one third of first generation family businesses surviving the succession process. It is the aim of this research to explore the contribution organisational culture has in the large failure rate of succession with family businesses. When successors enter into the organisation, they bring with them different perspectives on managerial issues suggesting that a change in leadership style will occur as a result of a succession. In answering the research questions, the researcher embarked on a two-phase research methodology utilising a quantitative and qualitative process. This triangulation process incorporates a self-administrated survey questionnaire, and six in-depth interviews. The survey questionnaire and interview schedules were structured using a combination of elements obtained from Harris's (1998) and Levinson's (1972). The survey data was analysed using various statistical methods, predominantly a Factor Analysis, where as Abstract iii the interviews were analysed using a theme retrieval process. The two processes were combined to yield the results. The findings of the research conclude that through the introduction of the successor in the change process, a new leadership style is introduced into the business. The new leader changes policies, practices and procedures, which are related to organisational climate and are the tangible aspects of organisational culture. Altering the climate essentially alters the culture, which may result in anxiety within the organisation and lead to tension. If these issues are not addressed, they may result in the demise of family businesses.

When successors enter into the organisation, they bring with them different perspectives on managerial issues suggesting that a change in leadership style will occur as a result of a succession. In answering the research questions, the researcher embarked on a two-phase research methodology utilising a quantitative and qualitative process. This triangulation process incorporates a self-administrated survey questionnaire, and six in-depth interviews. The survey questionnaire and interview schedules were structured using a combination of elements obtained from Harris's (1998) and Levinson's (1972). The survey data was analysed using various statistical methods, predominantly a Factor Analysis, where as Abstract iii the interviews were analysed using a theme retrieval process. The two processes were combined to yield the results. The findings of the research conclude that through the introduction of the successor in the change process, a new leadership style is introduced into the business. The new leader changes policies, practices and procedures, which are related to organisational climate and are the tangible aspects of organisational culture. Altering the climate essentially alters the culture, which may result in anxiety within the organisation and lead to tension. If these issues are not addressed, they may result in the demise of family businesses.

The question asked is: What is the experience of the labelled individual, and how does the label function as a ‘scientific fact’ (Kiesler, 2000) suffused within his being? In answering these questions, the researcher aims to abandon his own expectations, as is fitting with the phenomenological method, and to devote his sympathies entirely to the subjective disclosures which, it is hoped, the participants will offer. On this point, an obvious tension exists insofar as expectation and hypothesis necessarily constitute the inception of any research endeavour; and so, the notion of a complete bracketing of assumption and anticipation seems methodologically vague. The explorative impetus within this dissertation aims towards an elucidation of the effect of psychiatric diagnosis on the labelled individual, in terms of the individual’s experience of being-with-others. The impact of the offering of the label upon the individual’s interpersonal and intersubjective presence will be explored so as to establish whether psychiatric labelling unfolds as a disconnection of the individual from his co-existence with others.

The findings are that once the participants cope with the uncertainty about the meaning of the onset of symptoms by defining themselves as ill in somatic terms, the participants use external social and treatment resources to cope with the onset of symptoms and being chronically ill with CFS. As a consequence of feeling stigmatised in relation to social and professional scepticism about initially being ill and subsequently, being chronically ill with CFS, the participants become uncertain about the meaning of having CFS. Coping shifts to using internal resources by adopting self-management practises. In this process, firstly, existing self-management shifts in such a way that the participants view themselves as recovering or recovered from CFS, and secondly, the participants come to the understanding that difficulties with self-management cause and maintain CFS. The findings are discussed to conclude that CFS may be a misdiagnosis of difficulties with self-management. CFS itself may not be an 'objective' disorder, but a constituent of social processes. Becoming diagnosed with CFS arises as a consequence of the search for meaning in relation to the lay and professional assumption that psychological illness does not constitute 'real' illness, operating at both the levels of popular society and the doctor-patient relationship. By gaining an understanding of this interaction between identity and sexuality, it adds to our knowledge of the social dynamics that contribute to the prevalence or lack of prevalence of HIV/AIDS within population groups. This study uses a social constructionist discourse analytic framework and aims to explore the construction of sexual knowledge by Southern African youth of Indian-descent. The findings indicate that the construction of sex is primarily one of risk and ambiguity. Additionally, the construction of sexual knowledge highlights the significance of gender differentials and the importance of agency and responsibility for sexual education. These constructions reinforce traditional educational roles that contribute to the construction of sex as risky and ambiguous. In addition, a social identity of Indian-ness and othering is used as a strategy to give meaning to the lack of parental responsibility with regard to sexual education. The use of social identity is seen as highlighting the importance of acknowledging the sexual values within which youth are embedded. This study concludes with possible ways to shift these constructions. For example, one of the conclusions suggests the implementation of an alternative school-based sexual education that acknowledges the sexual values in which youth are embedded. Furthermore, this acknowledgement of sexual values should take place within a holistic sex education programme that is positive about sexuality. Additionally, a reframing of youth as capable and active decision-makers in their sexual education is necessitated in order to see youth as a potential resource in HIV/AIDS prevention.
