The association between HIV tri-therapy with the development of Type-2 Diabetes Mellitus in a rural South African district: A case-control study

- Description: Combination antiretroviral drugs(cARVs)prolong patients’ lives but are unfortunately thought to increase complications related to metabolic disorders including type-2 Diabetes Mellitus(DM).We sought to confirm the association of cARVs with type-2DM and ascertain the extent of this association in a rural South African setting. A case-control study of 177 (33.33%) cases with HIV/AIDS and type-2 DM were selected and compared with 354 (66.67%) non-DM HIV/AIDS unmatched controls from a rural district of South Africa’s third most populous province (Eastern Cape). Cases were identified from community health centres using the district health information system, and controls were identified using simple random sampling from the same health facilities. Odds Ratios (OR), together with 95% confidence intervals, were calculated for all the univariable and multivariable logistic analyses. This study found that cARVs significantly increased the occurrence of type-2 DM among HIV patients. Patients on protease inhibitors (PIs) were at least 21 times significantly more likely to be diabetic than those on the fixed dose combination (FDC); those on stavudine (D4T) and zidovudine (AZT) were 2.45 times and 9.44 times respectively more likely to be diabetic than those on FDC. The odds of diabetes increased by more than three-folds for those who had been on antiretroviral drugs for more than 6 years.

Using iterative learning to improve understanding during the informed consent process in a South African psychiatric genomics study

- Description: Obtaining informed consent is a great challenge in global health research. There is a need for tools that can screen for and improve potential research participants’ understanding of the research study at the time of recruitment. Limited empirical research has been conducted in low and middle income countries, evaluating informed consent processes in genomics research. We sought to investigate the quality of informed consent obtained in a South African psychiatric genomics study. A Xhosa language version of the University of California, San Diego Brief Assessment of Capacity to Consent Questionnaire (UBACC) was used to screen for capacity to consent and improve understanding through iterative learning in a sample of 528 Xhosa people with schizophrenia and 528 controls. We address two questions: firstly, whether research participants’ understanding of the research study improved through iterative learning; and secondly, what were predictors for better understanding of the research study at the initial screening? During screening 290 (55%) cases and 172 (33%) controls scored below the 14.5 cut-off for acceptable understanding of the research study elements, however after iterative learning only 38 (7%) cases and 13 (2.5%) controls continued to score below this cut-off. Significant variables associated with increased understanding of the consent included the psychiatric nurse recruiter conducting the consent screening, higher participant level of education, and being a control. The UBACC proved an effective tool to improve understanding of research study elements during consent, for both cases and controls. The tool holds utility for complex studies such as those involving genomics, where iterative learning can be used to make significant improvements in understanding of research study elements. The UBACC may be particularly important in groups with severe mental illness and lower education levels. Study recruiters play a significant role in managing the quality of the informed consent process.

Dynamic changes of soil surface organic carbon under different mulching practices in citrus orchards on sloping land

- Description: Publisher version , Mulching management has been used in many places all over the world to improve agricultural sustainability. However, the cycling of carbon in the soil under applications of mulch on sloping arable land is not yet fully understood. A four-year field experiment was carried out in Xiaofuling watershed of Danjiangkou reservoir in China. The object was to evaluate the effects of the application of straw mulch (ST) and grass mulch (GT) on dynamic changes in soil organic carbon and its fractions. Results showed that mulch applied on the soil surface increased the contents of SOC and its active fractions in the soil. Compared to the control without cover (CK), ST and GT treatments increased the contents of SOC, LOC, DOC, POC and EOC by 14.73%, 16.5%, 22.5%, 41.5% and 21%, respectively, in the 0–40 cm soil layer, and by 17%, 14%, 19%, and 30%, respectively, in the 0–100 cm soil layer. The contents of organic carbon and its active fractions decreased with increasing soil depth in all of the treatments. SOC was accumulated in the period of December to the following March. The contents of soil DOC and LOC were high in January to March, while the contents of soil POC and EOC were high in June to September. The relative contents of soil organic carbon fractions were POC > EOC > LOC > DOC over the four years. Straw mulching had no significant effect on the changes in soil organic carbon active fractions during the different periods. Based on this long-term field experiment in Danjiangkou reservoir, we found that straw mulching had a significant effect on soil, increasing SOC content and stock in slopping arable land, and that live grass mulching was more effective than rice straw mulching. We discuss possible optimal periods for the implementation of mulching practices on sloping land.

- Description: publisher version , From Introduction: The world faces a food security challenge with approximately 868 million people undernourished and about two billion people suffering from the negative health consequences of micronutrient deficiencies [1]. Yet, at least one-third of food produced for human consumption is lost or wasted along the food chain between farm and fork [2–7]. Food waste refers to wholesome edible material intended for human consumption, arising at any point in the food supply chain that is instead discarded, lost, degraded or consumed by pests [8]. Food loss is defined as the decrease in food quantity or quality which makes it unfit for human consumption [9]. According to the European Commission [10], food waste is composed of raw or cooked food materials such as vegetable peelings, meat trimmings and spoiled or excess ingredients or prepared food as well as bones, carcasses and organs. However, food waste can be measured only for edible products that are directed to human consumption [6]. Food losses take place at production, postharvest and processing stages in the food supply chain and the food losses that occur at the end of the food chain (retail and final consumption) are called “food waste” [11]. In our study food waste refers to food losses that occur at the end of the food chain (final consumption at household or consumer level) which include edible products that are directed to human consumption and are discarded when not consumed for various reasons.
