- Description: This thesis is a social history of malaria in southwestern Nigeria. It contributes to the burgeoning literature in the historiography of medicine, specifically the medicine and empire debate. Key to the issues raised in this thesis is the extent to which the limitations in colonial medical policies, most especially malaria control programmes, inspired critical and ingenious responses from African nationalists, doctors, patients, research volunteers, and indigenous medical practitioners. Challenged by a wide range of diseases and a paucity of health facilities and disease control schemes, African rural dwellers became medical pluralists in the ways they imagined and appropriated ideas of Western medicine alongside their indigenous medical practices. Beginning with a detailed historical exploration of the issues that informed the introduction of curative and preventive medicine in southwestern Nigeria, this thesis reveals the focus of colonial medicine. It exposes the one-sided nature of medical services in colonial spaces like southwestern Nigeria and the ways it shaped multifaceted responses from Africans, who were specifically side-lined till the 1950s when the rural medical service scheme was introduced. The focus of colonial medicine is drawn from relatively rich but often subjective historical evidence, such as a plethora of official reports of the department of medical and sanitary services, official correspondences within the colonial government in Lagos and Nigeria, and between the colonial government and the colonial office in the United Kingdom. Details of African responses to medical policies were garnered from newspaper publications and correspondences between the African public and the colonial government in Lagos. They reveal very interesting details of the ways Africans imagined, reimagined, and appropriated malaria control ideas and schemes. The central argument in this thesis is that attempts to control malaria in southwestern Nigeria till the 1950s, were shaped by a single concern to ameliorate the implications of the disease on the colonial state. It argues that this one-sided nature of malaria control programme informed the basis for medical pluralism in most rural spaces where African communities became patrons and sponsors of Western medicine and at the same time custodians of their indigenous medical practices. The series of justifications for the sustenance of these services were reinforced on the basis of the failure of the colonial state to guarantee the health needs of their colonial subjects. The aim of the thesis is to reinforce arguments that portray colonial medicine as a “tool of empire” but goes a bit further to explain the extent to which Africans related to this reality. It states quite categorically that Africans were not docile and silent, but that they acted decisively in ways that suited their varied interests and courses.

- Description: The question of ethnicity has been one of the most topical subjects of study by social scientists. The controversies around this phenomenon seem to have been heated up by the high visibility of mobilized and politicized ethnic groups in most multi-ethnic states. Therefore, the extent to which ethnic nationalities are able to effectively manage the interplay of ethnic differences determines to what extent a multi-ethnic nation develops without crisis. Historically Nigeria has come a long way from multi-ethnic entity with political differences and background to the amalgamation of 1914 till the present structure of thirty-six states. Ethnicity, no doubt has contributed immensely to ethnic conflicts in Nigeria because of long standing revulsion or resentments towards ethnic groups different from one’s own or fear of domination which can as well lead ethnic groups to resort to violence as a means to protect and preserve the existing ethnic groups. Significantly ethnicity in Nigeria, is a product inequality among the various ethnic groups orchestrated by a long period of colonialism; a period which witnessed the ascendancy of three major ethnic groups to the socio-political domination of other ethnic groups and a period when the three major ethnic groups were used as a pedestal for the distribution of socio-political goods, resulting in the inability of other ethnic groups to access these socio-political goods. This situation has continued to impact negatively on the forces of national integration and cohesion in ethnically divided Nigeria. Considering the relationship between ethnicity and development; socio-political exclusion is not only ethically dangerous to development but also economically unproductive. It deprives groups and individuals of the opportunity for the necessary development that can be beneficial to the society. Thus, it is important to develop an integrative socio-political frame-work that explicitly recognizes the participatory role of every ethnic group in governance. Hence, there is a need for the adoption of inclusive governance to manage ethnicity in Nigeria. Notwithstanding, ethnic conflict still persists and an attempt will be made in this study to identify the reasons. Central to socio-political sustainability in Nigeria is a system that should recognize that differences are important to development and encompass notions of equality. Such a system should acknowledge the socio-political and economic power of every ethnic group and promote a system devoid of ethnocentric and exclusionary socio-political and economic policies.

In vivo toxicological evaluation of peptide conjugated gold nanoparticles for potential application in colorectal cancer diagnosis

- Description: Colorectal cancer (CRC) is among the leading cause of cancer-related deaths in South Africa and worldwide. Efforts are being made at finding improved diagnostic tools, as early detection (before metastasis) is a major factor in CRC treatment. Colonoscopy is the most reliable detection method, but is a specialised and expensive procedure, which is invasive, not readily available and not patient-friendly. There is a risk of developing interval cancers, as colonoscopies are performed every 10 years after the age of 40. The development of non-invasive, cost efficient and readily available diagnostic tools to CRC, which can be performed at more regular intervals, using tumour-targeting molecular imaging agents, is of urgent attention. Gold nanoparticles (AuNPs) possess several physicochemical properties, including ease of synthesis, biocompatibility, and the ability to be conjugated by ligands or biomolecules such as polyethylene glycol (PEG) and peptides for improved stability, tissue targeting and selectivity. These factors potentiate the role in biomedical applications, including cancer theranostics. Conjugation of AuNPs with a targeting molecule (e.g. antibody or peptide) is directed against cancer cell receptors. The peptides, p.C, p.L, and p.14, bind to CRC cells in vitro. Conjugation of AuNPs with these peptides should be investigated for CRC diagnosis in vivo, as it is hypothesised to allow examinations at shorter intervals through imaging techniques. This could reduce the risk of interval cancers, but before developing this novel tool, in vivo toxicity evaluations are essential. This study was therefore aimed at investigating the short- and long-term toxicological effects of a single intravenous injection of peptides (p.C, p.L, and p.14) conjugated to AuNPs in a healthy rat model. Citrate-capped AuNPs were synthesised by the citrate-reduction method, and conjugated with each peptide (biotinylated) using a combination of PEG (99% PEG-OH and 1% PEG-biotin) as a stabilising agent and linker, via biotin-streptavidin interaction. Healthy male Wistar rats were intravenously injected with 14 nm citrate-AuNPs, PEG-, p.C-PEG, p.L-PEG, and p.14-PEG-AuNPs (100 μg/kg body weight), and the control rats were injected with phosphate buffered saline. The animals were monitored for behavioural, physiological, biochemical, haematological and histological changes, as well as inflammatory responses. Phase 1 rats were sacrificed 2 weeks post-injection to determine the immediate or acute toxicity of the AuNPs, while phase 2 animals were sacrificed 12 weeks post-injection, to investigate the delayed or persistence toxicity of the AuNPs. Results revealed no significant toxicities with the citrate, PEG-, p.C-PEG and p.14-PEG-AuNPs over 12 weeks post-exposure, as evidenced by biochemical assays such as serum marker enzymes, liver and kidney function markers, and cholestatic indicators; haematological parameters; oxidative stress markers; and histopathological examinations. P.L-PEG-AuNPs, however, caused significant toxicity (p<0.05) to rats, as evidenced by increased relative liver weight, increased malondialdehyde levels, and total white blood cell counts 2 weeks post-exposure when compared to the control group. This was, however, reversed during the 12 weeks post-exposure. Further, there were no evidence of inflammatory responses, using pro-inflammatory markers including phospho interleukin 18 (IL-18) and interferon-γ (IFN-γ), as indicated by immunohistochemical staining of the liver, spleen, kidney and colon of rats 2 weeks post-injection of AuNPs. Citrate, PEG-, p.C-PEG, and p.14-PEG-AuNPs did not induce immediate, acute or persistent toxicity, while p.L-PEG-AuNPs induced a transient acute toxicity. It can be concluded that 14 nm spherical citrate-AuNPs at 100 μg/kg body weight is a good candidate for biomedical applications, and as a suitable carrier for diagnostic and/or therapeutic molecules. Combination of 99% PEG-OH and 1% PEG-biotin is an appropriate option for stabilising AuNPs in biological environment, and conjugating secondary diagnostic or therapeutic biomolecules or agents to citrate-capped AuNPs. Peptide-conjugated AuNPs are suitable for the development into a diagnostic tool for CRC in vivo.

The implementation of inclusion policy for learners with special education needs in primary schools in Fort beafort district :Towards a rights based approach to education

- Description: The implementation of inclusion policy for learners with special education needs as stipulated in the Education White Paper 2001has been an issue of concern for South African schools. This study sheds light on the implementation of inclusion policy for learners with special education needs in the selected primary schools in the Fort Beaufort District in the Eastern Cape Province in South Africa. The aim of the study is to examine the implementation of inclusion policy for learners with special education needs. The study was placed within the post-positivism paradigm and used a mixed method research approach that integrated concurrent procedures in the collection, analysis and interpretation of the data. Questionnaires were used to collect quantitative data while qualitative data were collected through interviews, and document analysis. A total number of 30 teachers completed the questionnaires. The researcher conducted interviews with ten principals and eight teachers. She also held interviews with four education district officials and one provincial official. Data were analysed by statistical and non-statistical procedures. The study revealed that the inclusion policy is being implemented in the selected schools. However, there are challenges that are being encountered. Firstly, some teachers have not received adequate training with regards to inclusive education policy. Staff development workshops put in place to address this challenge are minimal, and there is not enough joint effort being established by the different stakeholders with regards to support and monitoring the implementation process. Despite the challenges that accompany implementation of inclusion of learners with special education needs, as well as the difficult working conditions, this study reveals that there are pockets of good practice of inclusion policy in some of the selected schools in the Fort Beaufort District. For instance, it was found that teachers implement inclusion of learners with special education needs, even though the majority of them do not have qualifications in special education. It was also revealed that teachers used learner-centred teaching methods to accommodate all learners, which include group and individual teaching, group demonstration, discussions, question-and-answer sessions, and practical activities.

This study sheds light on the implementation of the LO curriculum in selected high schools in the Fort Beaufort District, in the Eastern Cape Province in South Africa. The study adopted a qualitative approach and used face-to-face interviews, focus group interviews, classroom observation and document analysis to collect the data. The purpose of the study was to investigate the implementation of the Life Orientation curriculum in high schools in the Fort Beaufort District. Six high schools from the six clusters in the Fort Beaufort District were purposively selected to form the focus of the study. The participants were six principals, six LO teachers and thirty-six learners. The study revealed that lack of adequate teaching and learning materials in schools affected the implementation of the curriculum. Inadequacy of teaching and learning materials affected both the teachers and learners as they sought to make up for the shortages of teaching and learning materials by sourcing for local materials like news paper and magazine. In some of the schools, teachers made photocopies of textbooks for learners which could be time consuming and added to the teachers‘ workload. The study found that some of the teachers were frustrated because of the lack of adequate teaching and learning materials in LO. The results also revealed that the majority of the teachers had the Advanced Certificate in Education (ACE LO) qualification and/or an undergraduate qualification in Psychology, which are basic requirement for teaching LO, while the others had general teaching qualifications in other subjects. It was found that many learners were positive about LO, but it could not be proven that learners meant all their positive responses as it seemed that learners‘ responses were too good to be true. However, the results showed that learners had at least been taught and were aware of the consequences of the social problems like teenage pregnancy, drug abuse and HIV/AIDS that affect them. Some of the challenges that teachers encountered in the implementation of the Life Orientation curriculum were also highlighted in this study. These include; lack of adequate training which some of the teachers complained did not allow them to handle some topics like career choice and religious education as they should. Some of the teachers also stated that their culture and belief did not permit them to share some LO topics with their learners. There was also the challenge of learners turning against things taught in LO class. The study found that the support and monitoring put in place for LO teachers are in the form of documents such as subject guides, textbooks and sometimes funds to take learners out on outdoor activities, extra teachers to lessen the teachers‘ burdens, advice, workshops and training. There is course moderation where teachers‘ and learners‘ files are marked. In some of the schools where monitoring is carried out, it starts with the Heads of Departments (HODs) and ends with the principal. Many of the schools do not monitor the teaching of LO because of the trust the principals have in the teachers and challenge of school size. Class visits is prohibited by South African Democratic Teachers‘ Union. The subject is not being handled properly despite its importance in helping learners to be adequately guided towards positive self-concept formation, the realisation of their potential, and enabling them to protect themselves from various forms of social violence and abuse, and this in turn would make the society safer.

This is associated with liver disease. An ethnobotanical survey of plants used for the treatment of alcohol-induced liver damage in Nkonkobe Municipality was conducted. During the survey and also from information gathered in the literature, Pelargonium reniforme Curtis, was prominently mentioned, among other plants, as the species used generally for the treatment of alcohol-induced liver damage. This project was designed to evaluate the effects of the plant on alcohol-induced liver damage, including its antioxidant and antimicrobial properties. It also involves safety evaluation studies to determine if the plant is safe for consumption. Studies using rats of the Wistar strain were carried out to determine the protective and curative effects of P. reniforme on alcohol-induced liver damage. Results obtained showed that the plant extract can protect the liver cells as well as enhance recovery from tissue damage. The plant also showed good antimicrobial and antioxidant activity and this further validates its use in the treatment of liver diseases. Safety evaluation studies of the extract were carried out by investigating the effects of the oral administration on some haematological and biochemical parameters in male Wistar rats. The results obtained from the study suggest that the plant extract is not toxic at the doses used and is therefore safe for medicinal uses. The results of the various bioassays carried out in this project have justified the traditional uses of P. reniforme for the treatment of alcohol-induced liver damage.

- Description: Considerable efforts have been made in the modification of existing antimalarial drugs, and the support of incentive programmes have led to a drastic decrease in malaria cases reported by WHO during the past 6 years. However, the development of drug resistance threatens the eradication of this deadly disease and has prompted research on the synthesis of novel antimalarial drugs. Our research has involved the design and synthesis of novel benzylated phosphonate esters as potential 1-deoxy-D-xylose-5-phosphate reductoisomerase (DXR) inhibitors. A series of amidoalkylphosphonate esters were obtained by reacting various 3-subsituted anilines and heterocyclic amines with chloroalkanoyl chlorides and reacting the resulting chloroalkanamides with triethyl phosphite using Michaelis-Arbuzov methodology. Benzylation of the phosphonate esters afforded a series of novel N-benzylated derivatives in good yields and these compounds were fully characterised by NMR and HRMS methods. Several approaches to the introduction of a benzyl group at the C-2 position of the phosphonate ester derivatives have been explored, leading unexpectedly to the isolation of unprecedented tetrahydrofuranyl derivatives. Studies towards the preparation of potential bi-functional PfDXR / HIV-1 RT inhibitors have also been initiated. Preliminary in silico docking studies of selected non-benzylated and benzylated phosphonated derivatives into the Pf-DXR active-site has provided useful insight into the binding potential of these ligands. Bioassays have revealed a very low toxicity for all the synthesised phosphonated compounds and a number of these ligands also exhibit a promising inhibitory activity against the Plasmodium parasite.

- Description: Stenotrophomonas maltophilia is increasingly emerging as an opportunistic pathogen of global concern. Due to its inherent resistance to several classes of antibiotics including carbapenems and its ability to acquire mobile resistance elements, treatment of infections caused by S. maltophilia is a constant challenge for clinicians. Trimethoprim-sulphamethoxazole (TMP-SMX) is the generally accepted antibiotic of choice for the treatment of infections caused by this organism, but resistance to the drug is increasingly being reported; hence, the need for alternative therapeutic options. In this study, the antimicrobial susceptibility profile of 110 commensal S. maltophilia isolates obtained from Nkonkobe municipality, Eastern Cape Province, Republic of South Africa was investigated. Twenty-one antibiotics including TMP-SMX and the newer fluoroquinolones; levofloxacin, gatifloxacin and moxifloxacin were included in the antibiotic panel. About 63.4 percent of the isolates were susceptible to TMP-SMX with a resistance rate of 28.2 percent. The fluoroquinolones were more effective with susceptibilities ranging from 76 percent to 94.7 percent. Resistance to the fluoroquinolones ranged from 1.3 percent to 2.7 percent. Levofloxacin was the most effective fluoroquinolone tested. Phenotypic dectection of extended spectrum β-lactamases (ESBLs) showed double disc synergy test (DDST) positivity in 59.5 percent of the isolates. Cefepime was the most sensitive indicator cephalosporin in the DDST with 77.3 percent of suspected ESBL-producing isolates showing cefepime-clavulanic acid synergy. Isolates exhibited nine different ESBL phenotypes, however, PCR amplification of the bla genes revealed four isolates that possessed genes belonging to the CTX-M group (CTX-M-1 and CTX-M-8 groups). ESBL genes are usually carried on mobile elements such as plasmids and transposons which may also bear genes that mediate resistance to aminoglycosides, tetracyclines, TMP-SMX and fluoroquinolones. ESBL positive isolates appeared more susceptible to the fluoroquinolones compared to TMP-SMX but there was no significant relationship between ESBL production and susceptibility to these drugs (p > 0.05). The newer fluoroquinolones are a possible alternative treatment option for S. maltophilia infections in this environment but further studies and clinical investigations are needed to determine the in vivo efficacy of these drugs.

- Description: Characterization studies on the genetic diversity among cultivated cowpea (Vigna unguiculata (L.) varieties are valuable tools to optimize the use of available genetic resources by farmers, local communities, researchers and breeders. Eight cowpea (Vigna unguiculata (L.) genotypes ( Vegetable cowpea, Ivory grey, Okhalweni, Fahari, Fahari dark, 97K-1069-8, IT93K-73h, and 129-3) were subjected to molecular, morphological and agronomical characterization. DNA amplification fingerprinting markers were used to evaluate the genetic diversity among the eight genotypes. Nine random arbitrary primers were used to screen the eight genotypes to assess their ability to reveal polymorphisms in cowpea, and seven of them were selected for use in characterizing the total sample. A total of 43 bands were generated which are all polymorphic. On the average, the primers generated a total of 6.1 polymorphic bands. The resulting data-matrix included 43 analysed bands with a total of 344 characters. Neighbour joining analysis was used to generate the dendrogram, clustering the genotypes into two groups at an agglomerate coefficient of 0.30 irrespective of their geographical origins. The results also showed the presence of significant differences in morphological and quality traits among the genotypes. Fahari yielded the highest concentration of crude protein (46.51 mg/mg dry leaf) while Vegetable cowpea yielded the lowest (24.41 mg/mg dry leaf). The influence of manure was also found to be effective by increasing the crude protein content of the genotypes as shown by Fahari dark with an average of 53.53 mg/mg dry leaf as opposed to 39.85 mg/mg dry leaf without manure application. Although some small clusters grouped accessions of the same growth habits, a general lack of agreement between clustering and morphological features was observed. It can therefore be concluded that the significant differences between the molecular genetic analysis using DAF-PCR markers, morphologic characters and yield traits can be important tools to identify and discriminates the different cowpea genotypes.

- Description: HSP70 is one of the most important families of molecular chaperone that regulate the folding and transport of client proteins in an ATP dependent manner. The ATPase activity of HSP70 is stimulated through an interaction with its family of HSP40 co-chaperones. There is evidence to suggest that specific partnerships occur between the different HSP40 and HSP70 isoforms. While some of the residues involved in the interaction are known, many of the residues governing the specificity of HSP40-HSP70 partnerships are not precisely defined. It is not currently possible to predict which HSP40 and HSP70 isoforms will interact. We attempted to use bioinformatics to identify residues involved in the specificity of the interaction between the J domain from HSP40 and the ATPase domain from the HSP70 isoforms from humans. A total of 49 HSP40 and 13 HSP70 sequences from humans were retrieved and used for subsequent analyses. The HSP40 J domains and HSP70 ATPase domains were extracted using python scripts and classified according to the subcellular localization of the proteins using localization prediction programs. Motif analysis was carried out using the full length HSP40 proteins and Multiple Sequence Alignment (MSA) was performed to identify conserved residues that may contribute to the J domain – ATPase domain interactions. Phylogenetic inference of the proteins was also performed in order to study their evolutionary relationship. Homology models of the J domains and ATPase domains were generated. The corresponding models were docked using HADDOCK server in order to analyze possible putative interactions between the partner proteins using the Protein Interactions Calculator (PIC). The level of residue conservation was found to be higher in Type I and II HSP40 than in Type III J proteins. While highly conserved residues on helixes II and III could play critical roles in J domain interactions with corresponding HSP70s, conserved residues on helixes I and IV seemed to be significant in keeping the J domain in its right orientation for functional interactions with HSP70s. Our results also showed that helixes II and III formed the interaction interface for binding to HSP70 ATPase domain as well as the linker residues. Finally, data based docking procedures, such as applied in this study, could be an effective method to investigate protein-protein interactions complex of biomolecules.

The complexes were characterized by conductivity measurement, solubility, melting point, UV-Vis, PL, FTIR (ATR), NMR, Cyclic and square wave voltammetry. Nine chelating ligands, comprising of six (6) tripodal chelating ligands, two (2) vinyl monodentate ligands and one (1) vinyl tridentate ligand, were used for the synthesis of ruthenium(II) complexes ATR-FTIR spectra of all the ruthenium(II) complexes measured as solid samples, exhibited fine resolution IR bands in region between 3473-3438 cm-1 of carboxylic group in H2dcbpy. The bands in the range 3040-2950 cm-1 were ascribed to C-H bond stretching for the CH3 groups. The coordination of NCS group in the sphere of ruthenium(II) complexes related to series I, IV, VI VII and one compound of series VIII was investigated by ATR-FTIR spectroscopy. Bands in the range of 2116-2106 cm-1 and 777-770 cm-1 are ascribed to the respective N=C and the C=S bond stretching vibration which confirms the N-coordination of the SCN group. For all the complexes, the stretching vibration of Ru-N bonding was between 466 and 411 cm-1 due to coordination of the nitrogen atoms of the ligands to ruthenium central metal atom. The introduction of the two vinyl monodentate ligands (vpdiinp and vpbpp) in the coordination sphere of [Ru(bdmpmar)(vpdiinp)(H2dcbpy)]2+, [Ru(bdmpmar) (vpbpp)(H2dcbpy)]2+, [Ru(H2dcbpy)2(N^)(NCS)]+, [Ru(H2dcbpy)2(N^)2]2+, [Ru(H2dcbpy)(N^)2(NCS)2], [Ru(H2dcbpy)(N^)(NCS)3]– and [Ru(vptpy)(H2dcbpy)(N^^^)]2+ respectively, all related to series II, III, IV, V, VI, VII and two compounds of series VIII ruthenium(II) complexes, has been studied using the 1H and 13C NMR spectroscopy techniques. The 1H NMR spectra of series II-VII and the two compounds of series VIII of the ruthenium(II) complexes show multiplets in the aromatic region above 6 ppm due to the presence of either vpdiinp or vpbpp ligand, situated in different magnetic environment. However, no splitting pattern was observed in series I and part of VIII complexes possibly due to the absence vinyl monodentate subunits (vpdiinp and vpbpp) in series I and one of compound in series VIII ruthenium(II) complexes show no signals of complex splitting patterns. Carbon-13 NMR spectra data of series I to VIII ruthenium(II) complexes show most resonance signals range in the aromatic region of (δ 116.54-199.63ppm) corresponding to the molecular formulation of ruthenium(II) complexes incorporating 4,4-dicarboxy-2,2’-bipyridine, bdmpmar, vptpy, vpdiinp or vpbpp and NCS ligands respectively, depending on the intrinsic ligand variations. Carbon-13 NMR spectra data of series I, IV, VI VII and one compound in VIII show resonance peaks within the range 130-135 ppm are ascribed to NCS ligand confirming the presence of N-coordinated thiocyanate. Cyclic voltammograms of series I-IV and VI-VIII complexes display ruthenium-based oxidative peaks and the pyridines ligand-based reductive peaks. The redox behavior of complexes 4-12, 14-16, 18-20, 24-26 and 30 is dominated by the Ru(II)/R(III) redox couple in region (E1/2 between 0.53 and 1.18) and the pyridines ligand-based redox couples in the region between (E1/2 between −0.25 and −1.45). The photophysical property studies of the Ru(II) complexes are determined through the acquisitions of the absorption spectra, which tends to have profound effect on the short circuit current of DSSC. The absorption maxima were tuned by the introduction and variation of six (6) tripodal chelating ligands, two (2) vinyl monodentate ligands and one (1) vinyl tridentate ligand. From the studies, the results show that series IV, V, VI, VII and VIII complexes of molecular formula [Ru(H2dcbpy)2(N^)(NCS)]2+, [Ru(H2dcbpy)2(N^)2]2+, [Ru(H2dcbpy)(N^)2(NCS)2]2+, [Ru(H2dcbpy)(N^)(NCS)3]2+ and [Ru(vptpy)(H2dcbpy)(N^^^)]2+ respectively, have higher and multiple local absorption maxima near-IR region than the complexes of series I, II and III of molecular [Ru(bdmpmar)(H2dcbpy)(NCS)]2,+, [Ru(bdmpmar)(vpdiinp)(H2dcbpy)]2,+, [Ru(bdmpmar)(vpbpp)(H2dcbpy)]2,+respectively.

Applying blockchain technology to aspects of electronic health records in South Africa: lessons learnt

- Description: The purpose of this study was to explore the applicability of blockchain technology as a viable alternative for the secure storage and distribution of electronic health records in a South African context. The adoption of electronic health records (EHRs) has grown over recent years. Electronic health records (EHRs) can be seen as electronic versions of patients’ medical history. EHRs promise benefits such as improving the quality of care, reducing medical errors, reducing costs, saving time, and enhancing the availability and sharing of medical records. Blockchain, in simple terms, could be seen as a distributed database controlled by a group of individuals. Blockchain technology differs from other distributed ledger technology by bundling unrelated data into blocks that are chained together in a linked-list manner, hence the name blockchain. Blockchain technology strives to provide desirable features, such as decentralization, immutability, audibility, and transparency. EHRs are traditionally constructed with a cloud-based infrastructure to promote the storing and distribution of medical records. These medical records are commonly stored in a centralized architecture, such as a relational database. The centralized architecture employed by EHRs may present a single point of failure. These kinds of failures may lead to data-breaches. The cloud-based infrastructure is effective and efficient from an availability standpoint. The increased availability of electronic health records has brought forth challenges related to the security and privacy of the patient’s medical records. The sensitive nature of EHRs attracts the attention of cyber-criminals. There has been a rise in the number of data breaches related to electronic health records. The traditional infrastructure used by electronic health records can no longer ensure the privacy and security of patient’s medical records. To determine whether blockchain is a viable alternative to these approaches, the main objective of this study was to compile a technical report on the applicability of aspects of blockchain technology to the secure storage and distribution of electronic health records. The study first conducted a literature review to gather background on the current state of electronic health records and blockchain technology. The results of the literature review were used to compile an initial report. Experiments were conducted with various aspects of blockchain technology to build a technical baseline and to ultimately validate the initial report. The insights gained from the experiments served to refine the initial report into a final technical report. The final deliverable of this study was to devise a technical report. The technical report serves as a generalized overview of the applicability of blockchain technology as a secure storage and distribution mechanism for electronic health records. The main topics covered by the technical report to outline the applicability of blockchain technology to EHRs are as follows: authentication, authorization, audit log, storage and transactions. The insights gained from the study illustrate that permissioned blockchain technology can enhance the traditional AAA security scheme employed by traditional EHRs. The AAA security scheme entails the use of certificate-based authentication and attributebased access control for authorization. Audit logs can be stored in a semi-decentralized architecture that can enhance the security and privacy of audit logs. Using blockchain technology for storing electronic health records might not be a viable alternative to traditional EHRs architecture. Blockchain technology violates certain privacy regulations as information is stored in a permanent manner. Furthermore, blockchain technology is not optimized for dealing with large volumes of data. However, blockchain technology could be used to store a cryptographic hash of electronic health records to ensure the integrity of records. Permissioned blockchain technology can enhance the EHRs transaction process by transacting health records in a peer-to-peer infrastructure. In doing so, the above-mentioned AAA security scheme can enhance the security, confidentiality, and integrity of electronic health records shared across organizational bounds.

The garden as a metaphor for paradise

- Description: In this half thesis the use of the garden as a metaphor for paradise has been explored. The English word “ paradise“ was derived from the Greek word “ paradeisos” which in turn was derived from the Old Avestana “ pairi-daeza,” meaning an enclosure. In Ancient Persia the concept applied to an enclosed garden in the modern sense of the word. For this reason the thesis begins with an examination of the development of the garden in this desert region. A more-or-less continuous chain of development in both the physical and allegorical nature of the garden is traced through history from these Ancient Persian beginnings to the height of Mughal architecture (epitomised by the Taj Mahal), by way of the Muslim expansion through Central Asia and Europe. While the core elements of garden design were set in Ancient Persian times, and recur throughout the period studied, the impact of Islam on the local Persian culture brought about a new development of allegorical meaning associated with the garden. This allegorical development reached its apex, too, in the Taj Mahal in which, it is argued, the metaphorical representation of paradise in the garden tomb was made astonishingly explicit. The research for this mini thesis was gathered from secondary sources, including many published books and academic papers, photographic and diagrammatic evidence of extant ancient gardens, and reproductions of carpet designs.

- Description: In this half thesis the use of the garden as a metaphor for paradise has been explored. The English word “ paradise“ was derived from the Greek word “ paradeisos” which in turn was derived from the Old Avestana “ pairi-daeza,” meaning an enclosure. In Ancient Persia the concept applied to an enclosed garden in the modern sense of the word. For this reason the thesis begins with an examination of the development of the garden in this desert region. A more-or-less continuous chain of development in both the physical and allegorical nature of the garden is traced through history from these Ancient Persian beginnings to the height of Mughal architecture (epitomised by the Taj Mahal), by way of the Muslim expansion through Central Asia and Europe. While the core elements of garden design were set in Ancient Persian times, and recur throughout the period studied, the impact of Islam on the local Persian culture brought about a new development of allegorical meaning associated with the garden. This allegorical development reached its apex, too, in the Taj Mahal in which, it is argued, the metaphorical representation of paradise in the garden tomb was made astonishingly explicit. The research for this mini thesis was gathered from secondary sources, including many published books and academic papers, photographic and diagrammatic evidence of extant ancient gardens, and reproductions of carpet designs.

- Description: The continued growth of world commerce has led to the advance of the permanent establishment principles. These principles are, however, constantly challenged by the developments of e-commerce. This thesis considers the taxing of a permanent establishment and the influence of e-commerce on the concept of a permanent establishment. In 2000, the Organisation for Economic Co-operation and Development (“OECD”) developed and introduced guidelines on how to deal with e-commerce in the context of a permanent establishment. Since the OECD guidelines on e-commerce were issued, the permanent establishment principles have come under further scrutiny. The latest development came about in 2013 with the release of the Base Erosion and Profit Shifting (“BEPS”) Action Plan. This Action Plan addresses the intention of the OECD to deal with the taxing of the digital economy. With the development of e-commerce and the result of e-commerce creating intangible boundaries between countries, the concept of a virtual permanent establishment has emerged. This has resulted in the need to tax a presence of an enterprise in a jurisdiction where no actual physical connection can be established. Various authors have made suggestions on how to ensure that an economy in which business is being carried on is correctly compensated for in the form of taxes. The source of income is the driving force for the imposition of taxation today. The main goal of this thesis was to explore the alignment of the concepts of a permanent establishment and e-commerce in the digital economy. This study therefore examined the concepts of both permanent establishments and e-commerce, and explored authors’ views and suggestions on how to deal with the inter-related effects of these two concepts. Fieldwork is perceived as one such method. Fieldwork helps pupils acquire and develop understanding of geographical concepts, skills, attitudes and values through their own efforts and involvement. Fieldwork approaches have tended to change with the changing paradigms resulting in the development of approaches that are more pupil and experience oriented. In the South African school geography curricula fieldwork has been explicit since 1985. However, research has shown that in most South African secondary schools fieldwork as a teaching strategy is only applied to a limited extent. Teachers have always used financial constraints and time limitations as explanations for their failure to use fieldwork in teaching geography. This study attempts to demonstrate how the local environment of any school can be used effectively for teaching and learning most aspects of the senior secondary school geography syllabus, thereby alleviating the problems of time and money perceived by teachers as the major constraints inhibiting their use of fieldwork. In order to illustrate the effectiveness of fieldwork in the local environment, this study incorporated an analysis of the current senior secondary school geography syllabus, the identification of potential fieldwork sites in the Umtata District and the development and implementation of three fieldwork units based on three of the sites identified. The analysis of the evaluations of the three fieldwork units by the researcher, the pupils and the non-participant observer revealed that fieldwork conducted in the local environment is highly effective, interesting and rewarding to pupils even when they have no prior experience of fieldwork.

- Description: This study focuses on the structure and process of forgiveness as experienced by individuals, from the East London and surrounding areas, who either suffered gross human rights violations or who are related to someone who suffered gross human rights violations during the Apartheid era. Those who participated in the study testified at the Truth and Reconciliation Commission and expressed forgiveness towards the perpetrators of the violations. The aims of the study were to reveal: the participants' structure cif forgiveness, i.e. how they define forgiveness; the process of forgiveness, i.e. the changes that took place from the time of the violation up until the participants forgave the perpetrators: and, the relation. if any. between the participants' structure and process of forgiveness. It is argued that mainstream Psychology has neglected to study forgiveness because the subject matter is incompatible with the natural scientific method. For this reason, the study was approached from a hermeneutical paradigm. This was motivated by its ability to explicate the meaning and content of phenomena. Unstructured qualitative interviews were conducted with the participants. Data was analyzed using a multi-layered process of progressively deeper interpretation, employing a reading guide technique. Results indicated that authentic forgiveness is an unconditional commitment on the part of victims and survivors to relate positively towards the perpetrators. The relationship should include non-bitterness, non-vengeance, unconditional love and respect for their human rights. Another significant dimension of the structure of forgiveness is the fact that the desire for the truth is not abandoned although forgiveness has taken place. Forgiveness also does not take away the effects of the violation. This means that one does not forget although forgiveness has been granted. Results further indicate that the forgiveness process is highly complex, individualized and not instantaneous. The individuals have to deal with various intrapersonal conflicts and anxieties as a result of the violation, before forgiveness is explored as an option, and before they can finally forgive. Significant interrelations between the structure and process of forgiveness were also identified.

Writers in the available literature, contend that the objective of performance appraisal policy is to review work performance and provide feedback to facilitate measures and programmes to improve employee performance. Cascio and Aginis (2011:73) write that performance appraisal is a systematic description of individual or group job-relevant strengths and weaknesses as an important part of a performance management system. The study firstly analyses the effectiveness of performance appraisal in evaluating the management of the utilisation of resources. Secondly, the appropriateness of the tool in assessing the capacity of employees and contributing towards facilitating programmes and training to develop employee effectiveness. Thirdly, the study also examined the perception of employees on the use of performance appraisal as a management tool in the Department of Water and Sanitation Regional Operation Office. The study adopted a mainly qualitative methodological approach and purposively selected a sample of 30 employees at the Southern Operation Region Office of the Department of Water and Sanitation in Port Elizabeth who completed the questionnaire for the study. The choice of the qualitative research methodology is due to the fact that the approach is well suited to the collection of in-depth and perceptual information and for discovering underlying motivations and perceptions of the employees as respondents. The general employees at the area chosen for the study is one of a relatively young employee profile. The average age of the employees is about 28. The findings revealed that competence, assessment and development, communication and the adequate training which employees received based on the performance appraisal all had an impact on employee performance in Department of Water and Sanitation. Furthermore, factors, such as employee training, competence and communication could not be ignored since they were rated to a moderate extent v by the employees and it also contributed to employee performance. The study findings specifically highlight the importance of effective communication between the employer and the employees in order to improve employee performance appraisal processes. The study recommends a set of guidelines which could be used in improving the current performance appraisal policy and further boost employee performance.

It has been asserted that Mathematics is expected to help in accelerating social, economic and technological progress of any society. The Primary school level is very important in any educational system and any lack at this level would permeate to other levels of the educational system. Hence, this dissertation examines the use of textbooks by teachers in teaching Mathematics at selected primary schools in East London Education district. The study adopted the interpretivist paradigm and employed the qualitative approach in investigating the phenomenon. The dissertation made use of phenomenology research design as it attempted to understand people’s (learners’ and teachers’) perceptions, perspectives and understandings of the use of textbooks in teaching Mathematics. Semi-structured interviews and observation were used to elicit information from the six mathematics teachers and three heads of department who were purposively selected. The data collected were analysed using thematic approach where themes were developed from the research questions after transcribing, sorting and categorizing them. The findings of the study demonstrated that the participants selected textbooks from the ones prescribed by the department that were written in the CAPS documents. They had no influence or decision on textbook selection. All the teachers believed that lesson plan is very important and it is a must to have a very detailed and comprehensive lesson plan that is well-aligned to the contents of CAPS document and the textbook. The study further revealed that, for effective delivery of lesson and planning, there is need for a practical use of textbooks by the teacher. The study concluded that one of the most fundamental issues to consider is the fact that, as good as the use of textbook for planning and delivery of lesson activities is, it can also be a problem if not carefully and professionally planned and used accordingly. The over-dependence on textbooks at the expense of other resources may cause ineffective learning and teaching of mathematics.

Auditory learners appreciate listening to the teachers and sit down close to the teachers in class. Visual learners like to see things physically during teaching. They learn by materials like charts, graphs, and pictures. Kinesthetic learners learn by doing. Students can prefer one, two, or three learning styles; all these are subject to the availability of instructional materials. The Primary school level is very important in any educational system and any lack at this level would permeate to other levels of the educational system. Hence, this thesis examines the Learning Styles, Availability, and Utilization of Instructional Materials as Correlates of Grade 6 Learners’ Mathematics Performance in Buffalo City. The study adopted the positivist paradigm and employed the quantitative approach in investigating the phenomenon. The thesis made use of correlational research design as it attempted to find out the effects of learning styles and instructional materials on learners’ performance in Mathematics. Stratified sampling was used to select participants. Stratified sampling is a process of dividing the sample frame into strata to obtain relatively homogenous subgroups; this gave us 1225 Grade 6 learners selected across 35 schools in Buffalo City as the sample of the study. Three instruments which are Students’ Learning Styles Scale (SLSS), Availability and Utilization of Instructional Materials Inventory (AUIMI), and Mathematics Achievement Test (MAT) whose reliability co-efficient are; 0.87, 0.78 and 0.89 respectively. The data collected were analysed using descriptive and inferential statistics. The findings of the study revealed among others; students learning style (visual, auditory, and kinesthetic) have a significant relationship on Grade 6 learners Mathematics performance (B=-.113, t=-3.886, p<0.05). Students learning style (∆R2=.012, ∆F (1, 1223) = 5.047, p < 0.05) emerged as the best predictor of Grade 6 learners Mathematics performance in Buffalo City, while the availability and utilization of instructional materials (∆R2 = .000), ∆F (1,223) = .018, P > 0.05), and gender (∆R2 = .000, ∆F (1,1223) = .036, p>0.05) emerged as better predictors. The study further iv revealed that, Grade six learners with visual learning style ( = 13.242, SD = 5.565) had the best performance in Mathematics at Buffalo City, followed by learners with auditory learning style ( = 12.996, SD = 3.883), and learners with Kinaesthetic learning style ( = 11.525, SD =3.800). The study concluded that one of the most fundamental issues to consider in improving Mathematics performance is the understanding of the learners’ learning styles and effective use of appropriate instructional materials for teaching Mathematics.

The thesis made use of correlational research design as it attempted to find out the effects of learning styles and instructional materials on learners’ performance in Mathematics. Stratified sampling was used to select participants. Stratified sampling is a process of dividing the sample frame into strata to obtain relatively homogenous subgroups; this gave us 1225 Grade 6 learners selected across 35 schools in Buffalo City as the sample of the study. Three instruments which are Students’ Learning Styles Scale (SLSS), Availability and Utilization of Instructional Materials Inventory (AUIMI), and Mathematics Achievement Test (MAT) whose reliability co-efficient are; 0.87, 0.78 and 0.89 respectively. The data collected were analysed using descriptive and inferential statistics. The findings of the study revealed among others; students learning style (visual, auditory, and kinesthetic) have a significant relationship on Grade 6 learners Mathematics performance (B=-.113, t=-3.886, p<0.05). Students learning style (∆R2=.012, ∆F (1, 1223) = 5.047, p < 0.05) emerged as the best predictor of Grade 6 learners Mathematics performance in Buffalo City, while the availability and utilization of instructional materials (∆R2 = .000), ∆F (1,223) = .018, P > 0.05), and gender (∆R2 = .000, ∆F (1,1223) = .036, p>0.05) emerged as better predictors. The study further iv revealed that, Grade six learners with visual learning style ( = 13.242, SD = 5.565) had the best performance in Mathematics at Buffalo City, followed by learners with auditory learning style ( = 12.996, SD = 3.883), and learners with Kinaesthetic learning style ( = 11.525, SD =3.800). The study concluded that one of the most fundamental issues to consider in improving Mathematics performance is the understanding of the learners’ learning styles and effective use of appropriate instructional materials for teaching Mathematics.

Constructivism purports to be a transformation of the traditional curriculum. As such this article starts with a brief overview of behaviorism: the scientific approach to education. The main tenets underlying constructivism, how constructivism guides educators to change their classroom practice, and the implications to science teaching have been reviewed. 2 RESEARCH PROJECT TWO (Empirical study): Being Constructive: College students' learning of work and heat as aspects of the energy concept based a constructivist approach. Abstract This study is an extension of a literature review on constructivism as an alternate teaching and learning approach discussed in research project one. It is an empirical study concerning the use of a learning module based on a constructivist approach to develop pre-service student teachers' understanding of work and heat as aspects of the energy concept. The data consisted mainly of transcripts of students' interviews, written responses to questionnaires designed in the form of a worksheet, and comments from non-participant observers and students. The results seem to suggest that a carefully designed learning module based on a constructivist teaching and learning approach may be a valuable tool in developing pre-service student teachers' understanding of work and heat. 3 RESEARCH PROJECT THREE (Empirical study): A College in transition: A case study of the readiness of a college in the Eastern Cape province to implement Outcomes-Based Education in an Education Development centre. Abstract Curriculum 2005 premised on Outcomes-Based Education is the new curriculum framework for South Africa. It signifies a paradigm shift in education from the traditional 'telling-listening' relationship between the teacher and the learner to one that emphasises leamer-centred approach to the teaching process. Teachers, though recognized as crucial to the educational transformation process in the country have also being identified as ill-equipped to meet the challenges posed by Outcomes-Based Education. This study starts with a brief overview of the South African curriculum and the main tenets underlying Outcomes-Based Education. The institutional conditions and whether the lecturers at a college in the Eastern Cape province perceive the need for a change in their classroom practice were also investigated. Bearing in mind the need for further research to validate the findings of this study, positive indicators that emerged from the study suggest the readiness of the college to implement Outcomes-Based Education at the proposed Education Development Centre.
