- Description: Psychoanalysis as it exists today is not constituted by a single theoretical framework describing pathology and indicating a specific set of interventions. Since Freud originally conceptualised psychoanalytic understanding of pathology and psychoanalysis as a mode of intervention, there have been many revisions and reformulations of his theory. This thesis has attempted to integrate some psychoanalytic ideas regarding personality formation, psychopathology and psychotherapeutic intervention (Fairbairn, 1952; Ogden, 1992, 1994), with interpersonal (strategic / structural) ideas regarding intervention (Minuchin, 1974; Sullivan, 1940, 1953, 1964). In order to do so, the thesis used the relational psychoanalytic perspective, as depicted by Aron (1996) and Mitchell and Aron (1999), as an overarching conceptual framework. The focus from these points of view is the patient's internalized relationship patterns and the therapist's participation in their repetition. It is held that internalized relationship patterns are not only based on, but can be changed by, lived experience. From this perspective, the goal of therapy is to enhance the patient's capacity to reflect and think about experience, and therefore, to communicate about it. This means a change in the patient's predominant mode of communication. Ogden's (1994) modes of communication were described. The thesis suggested that Ogden's modes of communication can be stretched or translated into the types of communication outlined by Langs (1978). This thesis aimed to explore the deliberate use of countertransference responses to facilitate communication in the beginning stages of therapy with patients functioning predominantly in the paranoid-schizoid mode (Ogden, 1992). Patients who operate in this mode are often unable to tolerate interpretation and therefore traditional approaches to intervention are not effective. A "strategic / structural relational psychoanalytic" approach to treatment was proposed. It was suggested that therapists utilize joining and accommodation techniques as described by Minuchin (1974) and alter their style of interaction to match that of the various object relational constellations that they have managed to identify within the patient via their countertransference responses. It was hypothesized that patients need their therapists to be similar to their original objects in order to feel safe in the therapeutic environment and that this may facilitate communication in the beginning stages of therapy. The research utilized a qualitative research approach. Qualitative research methods attempt to use data gathered phenomenologically, always acknowledging the researcher's biases when gathering the data. The data gathered is then interpreted according to various theories or hermeneutic lenses. The hypothesis mentioned above has been investigated by analyzing three cases in terms of the research questions based on Langs' (1978) classification of communication. The thesis described the difficulties inherent in collecting clinical data from psychologists working from within a psychoanalytic framework. Eventually three sets of therapy details and verbatim therapy transcripts were obtained, provided in the thesis and analyzed in terms of the research questions. However evidence for the success of the hypothesized alternate approach was not found in this research study. It was suggested that other possible methods might be useful to investigate the hypothesized approach further.

- Description: Traditional medicine has a long history of being used for treating various ailments ranging in severity. Although traditional medicine has typically been the health care for the poorest levels of society, there is a worldwide growth in popularity. The growing popularity of traditional medicine, termed the green boom, may be ascribed to people taking a more holistic approach to maintain their health. Traditional medicine is widely used on a regular basis by 70% of South Africans. Various indigenous medicinal plants are used for the preparation of traditional herbal medicine. These plants are mostly indigenous to the regions were it is used. In this study four medicinal plants (Bulbine frutescens, Leonotis leounurus, Melianthus major & Zantedecshia aethiopica) that are traditionally used in the Eastern Cape region for treating burn wound infections, were collected for investigation. The in vitro antibacterial activity of these plants was tested against different bacterial strains of eight different bacteria. The bacteria used in this investigation included bacterial strains of four Gram-positive bacteria, S. aureus, methicillin-resistant S. aureus (MRSA), E. feacalis, S. pyogenes and four Gramnegative bacteria, P. aeruginosa, A. baumanii, K. pneumoniae and P. mirabilis. Traditional preparations as well as three different extracts (methanol, aqueous & acetone) of the plants were used for in vitro antibacterial activity testing. The microtitre plate assay and agar dilution assay were used for determining the antibacterial activity of the traditional preparations and plant extracts against the different bacterial strains. In the microtitre plate assay the antibacterial activity was tested using the bacterial growth indicator, INT and a microtitre plate spectrophotometer to determine the minimal inhibitory concentrations of the plant extracts and traditional preparations. The microtitre plate assay was used for testing the antibacterial activity of the plants against the bacterial strains of five bacteria, S. aureus, MRSA, P. aeruginosa, A. baumanii and K. pneumoniae. The bacterial strains of the three bacteria, S. pyogenes, E. feacalis and P. mirabilis were not compatible with the microtitre plate assay using INT and spectrophotometric readings to determine bacterial inhibition. Therefore the agar dilution assay were used as an alternative method for determining the MIC’s of the plant extracts against the bacterial strains of these bacteria. The initial plant extract concentration in the microtitre plate assay differed with the different plant extracts in the microtitre plate assay. Acetone followed by methanol extracted the highest plant extract concentrations with the different medicinal plants. M. major followed by L. leonurus produced the highest plant extract concentrations following extraction with the different extraction solvents. Consequently the acetone extract of M. major had the highest plant extract concentration before serial dilution in the microtitre plate assay. Uniform plant extract concentrations were tested in the agar dilution assay. The methanol extract followed by the acetone extract of the plants gave the highest antibacterial activity against the different bacterial strains. The extracts of M. major followed by L. leonurus inhibited the highest number of bacterial strains in the microtitre plate assay and the extracts of B. frutescens inhibited the lowest number of bacterial strains. The acetone and methanol extracts of M. major were the only extracts that displayed antibacterial activity in the agar dilution assay. Tertiary treatment in integrated algal ponding systems

- Description: Inadequate sanitation is one of the leading causes of water pollution and consequently illness in many underdeveloped countries, including South Africa and, specifically, the Eastern Cape Province, where cholera has become endemic. As modern wastewater treatment processes are often energy intensive and expensive, they are not suitable for use in these areas. There is thus a need to develop more sustainable wastewater treatment technologies for application in smaller communities. The integrated algal ponding system (IAPS) was identified as a possible solution to this wastewater management problem and was investigated for adaptation to local conditions, at the Rhodes University Environmental Experimental Field Station in Grahamstown, South Africa. The system was monitored over a period of nine years, with various configuration adjustments of the high rate algal pond (HRAP) unit operation investigated. Under standard operating conditions, the system was able to achieve levels of nutrient and organic removal comparable with conventional wastewater treatment works. The mean nitrate level achieved in the effluent was below the 15mg.l-1 South African discharge standard, however, nitrate removal in the IAPS was found to be inconsistent. Although the system was unable to sustain chemical oxygen demand (COD) removal to below the 75mg.l-1 South African discharge standard, a removal rate of 87% was recorded, with the residual COD remaining in the form of algal biomass. Previous studies in the Eastern Cape Province have shown that few small wastewater treatment works produce effluent that meets the microbial count specification. Therefore, in addition to the collation of IAPS data from the entire nine year monitoring period, this study also investigated the use of the HRAP as an independent unit operation for disinfection of effluent from small sewage plants. It was demonstrated that the independent high rate algal pond (IHRAP) as a free standing unit operation could consistently produce water with Escherichia coli counts of 0cfu.100ml-1. The observed effect was related to a number of possible conditions prevailing in the system, including elevated pH, sunlight and dissolved oxygen. It was also found that the IHRAP greatly enhanced the nutrient removal capabilities of the conventional IAPS, making it possible to reliably and consistently maintain phosphate and ammonium levels in the final effluent to below 5mg.l-1 and 2mg.l-1 respectively (South African discharge standards are 10mg.l-1 and 3mg.l-1 in each case). The quality of the final effluent produced by the optimisation of the IAPS would allow it to be used for irrigation, thereby providing an alternative water source in water stressed areas. The system also proved to be exceptionally robust and data collected during periods of intensive and low management regimes were broadly comparable. Results of the 9 year study have demonstrated reliable performance of the IAPS and its use an appropriate, sustainable wastewater treatment option for small communities.

The development of best practice guidelines for the contingency management of health-related absenteeism in the motor manufacturing industry

The theoretical study focused on the management of absenteeism, wellness, ill-health/mental problems and HIV/AIDS, as well as contingency strategies aimed at maintaining production and service provision. iii The findings from the literature study were integrated into a model of best practices for the contingency management of health-related absenteeism. This model was used as a basis for the development of a survey questionnaire to determine whether senior human resources practitioners, occupational health practitioners or line managers, who were responsible for the management of health-related absenteeism in organisations, agreed with the best practice guidelines developed in the study. The survey was conducted in the motor and motor component industry in the Nelson Mandela Metropolitan Municipality and Buffalo City Metropole. The empirical results from the study showed a strong concurrence with the best practices guidelines developed in the study, with the exception of the strategies aimed at maintaining undisrupted production and service provision during periods of high absenteeism. In particular, disagreement was shown with regard to alternative work arrangements such as flexible work-hours, a compressed workweek, telecommuting and job-sharing. Absenteeism, in general, is an issue that organisations are challenged with on a daily basis. The proliferation of various diseases, specifically HIV/AIDS, is contributing to this problem. An integrated and strategic approach is required to deal effectively and constructively with the immediate and expected future impact of health-related issues on absenteeism. The research is presented methodologically as an action research located within two paradigmatic positions, interpretive and critical. The research attempted to understand, interpret and critique the process of curriculum negotiation within the context of teacher education in South Africa In order to understand the negotiated curriculum process, experiences of the participants are presented through the use of their voices within the thesis. The interpretation is based on the construction and reconstruction of meaning during the enactment of the negotiated curriculum process and during the writing and reviewing of this thesis. The critique is rooted in the historical, cultural and social contexts of both the students and the author. The main contention of this thesis is that curriculum negotiation is not necessarily a suitable vehicle for developing a critical pedagogy in pre-service teacher education when all the participants form a homogenous group in this case, white middle-class women. The democratic values promoted within the context of our curriculum negotiation were fraught with dilemmas and entrenched the values of western liberalism. At most, the curriculum negotiation process and the development of a democratic learning environment, promoted a conscientisation at an individual level, namely a ' transformation of consciousness' . The democratic values promoted in our pedagogy were not sufficient in bringing about social change, a 'transformation for social action'.

The lack of use of a sophisticated risk assessment model is also disappointing, with Cadbury ignoring individual project risk and adopting a naive approach. Finally, this study indicates that Cadbury managers need not be forced into choosing either an economic/normative approach or a strategic/managerial approach to capital-investment decision-making but that rather a hybrid approach, including both the economic and strategic dimensions of choice, is more applicable for effective strategy incorporation.

- Description: This thesis deals with the conceptualization of access to land by the dispossessed as a human right and commences with an account of the struggle for land between the peoples of African and European extractions in South Africa. It is observed that the latter assumed sovereignty over the ancestral lands of the former. The thesis discusses the theoretical foundation of the study and situates the topic within its conceptual parameters. The writer examines the notions of justice and equity in the context of the post apartheid constitutional mandate to redress the skewed policy of the past. It is argued that the dispossession of Africans from lands that they had possessed for thousands of years on the assumption that the land was terra nullius was profoundly iniquitous and unjust. Although the study is technically limited to dispossessions occurring on or after the 13th June 1913, it covers a fairly extensive account of dispossession predating this date. This historical analysis is imperative for two reasons. Besides supporting the writer’s contention that the limitation of restitution to land dispossessed on or after 1913 was arbitrary, it also highlights both the material and non-material cost of the devastating wars of dispossessions. The candidate comments extensively on the post apartheid constitutional property structure which was conceived as a redress to the imbalance created by dispossession. This underlying objective explains why the state’s present land policy is geared towards facilitating access to land for the landless. The thesis investigates the extent to which the present property structure which defines access to land as a human right has succeeded in achieving the stated objective. It reviews the strengths and weaknesses of the land restitution process as well as the question of the payment of just and equitable compensation for land expropriated for restitution. The latter was carefully examined because it plays a crucial role in the success or otherwise of the restitution scheme. The writer argues that the courts have, on occasions, construed just and equitable compensation generously. This approach has failed to reflect the moral component inherent in the Aristotelian corrective justice. This, in the context of South Africa, requires compensation to reflect the fact that what is being paid for is land dispossessed from the forebears of indigenous inhabitants. It seems obvious that the scales of justice are tilted heavily in favour of the propertied class whose ancestors were responsible for this dispossession. This has a ripple effect on the pace of the restitution process. It also seems to have the effect of favouring the property class at the expense of the entire restitution process. The candidate also comments on the court’s differing approaches to the interpretation of the constitutional property clause. The candidate contends that the construction of the property clause and related pieces of legislation in a manner that stresses the maintenance of a balance between private property interest and land reform is flawed. This contention is supported by the fact that these values do not have proportional worth in the present property context of South Africa. The narrow definition of “past racially discriminatory law and practices” and labour tenant as used in the relevant post apartheid land reform laws is criticized for the same reason of its uncontextual approach. A comparative appraisal of similar developments relating to property law in other societies like India and Zimbabwe has been done. The writer has treated the post reform land evictions as a form of dispossession. The candidate notes that the country should guard against allowing the disastrous developments in Zimbabwe to influence events in the country and calls for an amendment of the property clause of the constitution in response to the practical difficulties which a decade of the operation of the current constitution has revealed.

- Description: Metronidazole is a synthetic, nitroimidazole-derivative antibacterial and antiprotozoal agent (ed. McEvoy, 2001). It has been reported that crystallization occurs in aqueous suspensions of metronidazole benzoate, a bland-tasting prodrug of metronidazole, as a result of conversion from the anhydrous to the monohydrate form, thereby compromising the stability and clinical efficacy of the substance due to the particle size growth (Hoelgaard & Moller, 1983). A generic South African based pharmaceutical company commenced formulation of an aqueous metronidazole benzoate suspension and experienced problems with crystallization that occurred in products stored at 2 to 8 °C. This study aimed to continue development of the product in order to identify a formulation that prevents formation of the hydrate form of metronidazole benzoate and the accompanying crystal growth. A variety of metronidazole benzoate suspensions were manufactured on a laboratory scale using a number of natural and synthetic suspending agents, including magnesium aluminium silicate, povidone K90, xanthan gum and Avicel® RC-591 (microcrystalline cellulose and carboxymethylcellulose sodium), over a range of concentrations. Analytical quantification methods were developed and validated, and the physicochemical properties of the raw material and finished products were fully characterized. Rheological tests were performed in order to characterize the suspension flow properties. Real-time and accelerated stability studies and a temperature cycle study were conducted in accordance with the International Conference on Harmonization (ICH) guidelines. Conversion of metronidazole benzoate to the monohydrate form took place in suspensions containing xanthan gum 0.65 percent m/v under real-time and accelerated storage conditions. The suspensions containing Avicel® RC-591 were found to be physically and chemically stable after the temperature cycle and over the 12-week period whilst stored at 25 ºC / 60 percent RH and 5 ºC. The suspensions were chemically stable whilst stored at 40 ºC / 75 percent RH but showed sedimentation at this accelerated condition. The metronidazole benzoate contained in these products remained in the anhydrous state under all storage conditions and were consequently concluded to be the most stable formulations out of all the products analyzed in the current study. The suspending agent system consisting of microcrystalline cellulose and carboxymethylcellulose sodium thus shows promise in preventing the conversion of metronidazole benzoate from the anhydrate to the monohydrate form, thereby inhibited the subsequent increase in particle size due to crystal growth.

It is proposed that for effective team-work to take place, an understanding of the roles and functions of team members is imperative in providing quality mental health care. This study explored health professionals’ perceptions of the role and function of clinical psychologists working as part of a health care team in a community context. Three focus groups and three individual interviews were conducted with social workers, nurses and doctors. The data was then processed and analysed using a grounded theory method. The research highlighted the importance of knowledge, and how this affects referrals, perceptions, inter-professional relations and the perceived usefulness of clinical psychology and clinical psychologists. What is of significance is the potential power that the health professionals have as gatekeepers between the general public and clinical psychologists.

An assessment of strategies used to retain nurses in selected hospitals in the Nelson Mandela Metropolitan Municipal area

- Description: The harsh realities currently facing public hospitals such as budget cuts, understaffing and poor working conditions, have resulted in nurses experiencing low morale and finding it increasingly difficult to stay in the public health sector. The obvious targeting of South African nurses by recruitment agencies with job offers to work overseas is causing a massive skills’ drain that the country cannot afford. The situation is further compounded by the fact that the government is freezing posts of those nurses who have resigned. Consequently, nurses who are currently working in these hospitals have to cope with unreasonable workloads. Public hospitals are nowadays faced with the challenge of delivering quality health care within the constraints of gross staff shortages, and an increasingly competitive global health environment. The main objective of this study was to identify strategies that can be implemented by hospitals in order to reduce turnover, thereby enhancing retention of staff. Interviews were conducted with the matrons-in-charge of the hospitals under study to investigate strategies that are in place to improve retention. Secondary data regarding staff turnover in the hospitals under study was collected and analysed. It emerged that there are no concrete strategies in place to lower turnover. Hospitals are indeed experiencing staff turnover and posts that were vacated by those nurses that have resigned are frozen. The literature survey revealed three strategies that can be utilised to retain employees, namely, motivation strategies, effective leadership strategies and human resources strategies. Based on various literature reviews, it was concluded that the three strategies are interrelated. The empirical survey was conducted at the three selected hospitals, to determine the extent to which the retention strategies identified in the literature survey are being utilised. All three are invariably used in the hospitals. It is evident that the leadership behaviour in these hospitals affects the level of motivation of employees. The extent to which some of the human resources strategies are utilised invariably leads to low morale and dissatisfaction among employees. Lastly, research findings were assessed with the aim of drawing conclusions so that appropriate recommendations could be made. Recommendations focused on the retention strategies that can be implemented to boost morale of employees and enhance retention.
